Job Overview:

The Bethesda Case Manager will be responsible for working with people supported, their team, and other qualified professionals to develop and maintain assigned individual’s supports. The Case Manager will be responsible for ensuring all aspects of the individual’s supports are in accordance with state and federal regulations. The Case Manager is responsible for leading quality reviews of individual’s supports and maintaining compliance with licensing standards.

What you'll be doing:

  • Provide support to individuals as assigned by Director group. If the person receives supports in both residential and vocational services lines, the Case Manager may be expected to provide services in both.
  • Completes all required Oregon ISP assessments, documentation, and supports unless task is assigned to nursing or outside qualified behavior professional.
  • Provides and maintains all qualifications needed to deliver professional behavior services to individuals on case load as outlined in Oregon Administrative Rules.
  • Advocates for and seeks innovative ways to provide necessary supports; assists people to achieve their desired outcomes, and champions their rights.
  • Assists the individual and team to schedule and facilitate ISP team meetings.
  • Works with the Program Manager and County to facilitate new admissions and discharges.
  • Completes all paperwork required by organization's policies and state regulations.
  • Acts as the liaison between individual, families, legal authorized representative, county, and team while documenting significant information shared with all.
  • Promotes self-advocacy and independence by encouraging each individual to identify needs and preferences; and when necessary, provides advocacy on behalf of each individual to ensure appropriate treatment, training and protection of rights.
  • Assists the Area Director with Quality Assurance audits.
  • When indicated, works with other professionals to complete assessments and make support recommendations.


What we want to see from you:

  • MA or MS in psychology, sociology, human services, education, and social work, with one year of experience using positive behavior support services with individuals who experience intellectual or developmental disabilities.

OR

  • BA or BS in psychology, sociology, human services, education, and social work with three years of experience using positive behavior support services with individuals who experience intellectual or developmental disabilities.
  • Must possess a valid driver's license and good driving record in last 3 years
  • Must be willing to undergo a background check and drug test as defined by regulations and policies
  • Must possess good verbal, reading, and written communication in English; basic math skills; and basic computer skills.
  • Must be able to travel.
  • Within one year of hire, be certified in an ODDS approved behavior curriculum.
  • Complete a minimum of 12 hours each biennium of ongoing education in the field of positive behavior support services, adaptive behaviors, behavior management, or a related topic.
  • Maintain a current approved criminal background check specific to their role as a behavior professional as described in OAR 407-007-0200 through 407-007-0370.
