Responsibilities

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

* Ensures quality and safe delivery of services to patients according to individualized care plan under the supervision of a Registered Nurse.

* Uses equipment and supplies safely, effectively, and efficiently

* Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served

* Possesses communication skills, including the ability to read, write, and verbally report clinical information to patients, caregivers, and hospice staff.

* Recognizes the influence of personal, spiritual, and cultural values on perceptions regarding death and dying and their impact on quality of life.

* Hospice Aide services will be ordered by the Interdisciplinary Group and included in the plan of care.

* Reports changes in the patient's medical, nursing, and social needs to a Registered Nurse as the changes relate to the plan of care.

* Completes appropriate documentation in an accurate and timely manner and in compliance with the hospice program's policies and procedures.

* Creates successful interpersonal relationships with patients/families while maintaining personal and professional boundaries.

* Demonstrates listening skills and offers comforting presence to patients/families.

* Completes a minimum of twelve (12) hours of continuing education programs (3 hours per quarter) or approved Net Learning offerings each year.

* Completes the Hospice Aide Competency Evaluation of skills each year with a satisfactory result.

* Provides the following essential care to patients under the direction of a Registered Nurse:

* Reading and recording temperature, pulse, and respiration.

* Basic infection control procedures.

* Maintenance of a clean, safe, and healthy environment.

* Uses appropriate and safe techniques in performing personal hygiene and grooming tasks that include:

* Bed bath

* Tub and shower bath

* Hair shampoo (sink, tub, and bed)

* Nail and skin care, including shaving

* Oral hygiene

* Toileting and elimination

* Measure and record intake and output

* Change bed linens

* Change simple, non-sterile dressings

* Launder clothing and bed linens

* Safe transfer techniques and ambulation

* Normal range of motion and positioning

* Adequate nutrition and fluid intake

* Hospice Relief Support

* Communicates with Interdisciplinary Team and participates in care coordination to achieve high quality care.

* Demonstrates accountability to patients, families, colleagues, and organization staff.

* Maintains regular and consistent attendance at work.

* Notifies Registered Nurse of any changes in patient's behavior or condition.

* Behaves in a manner consistent with Mission, Vision, Values and Expectations for Excellence.

* Maintains compliance with OSHA, Accreditation Standards and Risk Management guidelines.

* Performs responsibilities within the aides scope of practice and maintain compliance with Personnel policies and procedures.

* Behaves in a manner consistent with all Corporate Compliance policies and procedures.

* Performs other duties as requested by the Clinical Manager to facilitate smooth and effective operations of the organization.

* Meets measures as determined and required for job productivity and performance improvement.

* Participates in quality assurance and performance improvement activities as assigned.

* Achieve certification in Integrated Care Management within 1 year of hire. Certification includes completing the Integrated Care Management formal training session, successfully completing assigned computer based-learning modules within 2 weeks of training, and a competency evaluation. Use the Integrated Care Management philosophy, skills, tools and documentation in clinical practice. Demonstrate competency in selected ICM skills annually.

* Employee is responsible for completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UPAH programs and initiatives.

Qualifications

QUALIFICATIONS REQUIRED:

* High School diploma or GED

* Completion of a Nursing Assistant Training Program of a minimum of 75 hours

* Valid Iowa or Illinois certification.

* Strong interpersonal skills

* Ability to work as a collaborative team member across service lines as position requires.

* Ability to understand and apply guidelines, policies and procedures.

* Age specific care and reporting of Dependent Adult and Child Abuse training with in one month of employment.

* CPR Certification.

* Valid Mandatory Reporter course completion by state(s) requirement

* Valid, licensed driver, with automobile insurance in accordance with state and/or organizational requirements

* Proficiency with computer applications (i.e. Excel, Word, Access

QUALIFICATIONS PREFERRED:

2-5 years hospice experience in a nursing or hospice facility

Completion of a 120 hour Nursing Assistant course and holds current State of Iowa Certification
