We are looking for an organized individual to join us in our mission to enhance the lives of aging adults and their families.  The position of Client Care Coordinator performs a variety of duties that relate to client care including care consultations with potential clients and family members, client/Caregivers introductions and quality assurance visits with existing clients.

Responsibilities
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan
  • Work with other team members to coordinate various aspects of a client's care
  • Create and maintain client records
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients
  • Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and Caregivers, and referral providers/care providers
Qualifications
  • Possess excellent verbal and written communication skills and the ability to listen effectively
  • Be service oriented and able to work well in a team environment in a fast-paced organization
  • Proficient with computer and various software programs
  • College degree preferred
  • At least 3 - 5 years Experience in home care, health care or senior-related industry preferred
