POSITION SUMMARY

Provides medical case management services with minimal supervision. Accepts case management referrals; maintains assigned caseload (125-150). Assesses patients healthcare needs and identifies community resources. Promotes and coordinates optimum healthcare services, with cost effective solutions.

The Medical Group Care Manager works predominantly within Practice Groups to deal with the increasing complicated chronic disease patient with multiple co-morbid conditions. The MGCM interfaces with physicians, nurses, all departments within the hospital facilities, and community resources to expedite medically appropriate cost-effective care. The MGCM applies clinical expertise and medical appropriateness criteria to ensure appropriate level of care, resource utilization and discharge planning. Serves as a patient advocate and assists in the identification and improvement of service delivery.

JOB REQUIREMENTS

1. Current RN license for the State of Virginia required.

2. Manages the case load of patients with chronic disease under the care the supervision of a physician.

3. For the patient improves healthcare access and promotes client knowledge and behavior change.

4. Facilitates the transition of care from post hospitalization, ER discharges, nursing home or assisted living settings. To reduce readmissions and prevent visits for care that would better be delivered in health centers or provider offices.

5. Facilitates the initiation of diagnostic services, treatment planning, and therapeutic treatment while the patient is enrolled in the program in the practice setting.

6. Collaborates with the Emergency Department physicians, Hospitalists and/or attending physicians to determine correct level of care at the time of admission or discharge from alternate care setting.

7. Collaborates in the utilization of clinical physician order sets for designated targeted diagnoses to ensure follow through on work assignments.

8. Consults with the medical staff, nursing staff, and ancillary department staff to eliminate barriers to the efficient delivery of care. Identifies service delivery problems and potential for effective case management intervention.

9. Develops and implements case management plans to maximize healthcare outcomes and facilitate wellness.

10. Maintains resource manual for case management program.

11. Assist medical group with coordinating care of the uninsured/unassigned patient and facilitates referrals into one of the Bon Secours Providers of Care.

12. Initiates communication with other care managers on admitted and observation patients to facilitate care.

13. Manages a clinical team with primary practice.
