Job Summary:
The Mobile Assessment Counselor provides face to face risk assessment, pre- commitment investigations, and crisis intervention primarily in a hospital setting both within Bourbon Community Hospital as well as in surrounding sister hospitals and partnered facilities.
Assessors coordinate the referral and intake process, determines appropriate level of care, facilitates the admission process and coordinates transfer to a higher level of care when necessary. The mobile Clinician maintains communication with referral sources, families, patients and referring site staff and provides supporting documentation for the patients' medical chart.
Essential Functions:
Conducts screening and/or assessments in keeping with clinical knowledge, skill level, and department policies.
Refer callers to appropriate resources
Demonstrates sufficient understanding of services available in the surrounding community.
Interacts well with referral sources
Facilitates prompt communications in the best interest of the patient and the facility's operations.
Demonstrates knowledge of programs amp/or unit services, policies, and procedures.
Responds to request for mobile within 15 minutes and presents to location within 60 minutes.
Appropriately logs and submits completed assessments in a timely manner.
Documentation on Intake Call Sheets is complete and correct in all areas.
Education:
Master's degree in Counseling, Psychology, Social Work, or Human Services field, or Licensed PsychiatricOR RN with a minimum of a BSN and psychiatric-mental health certificationCertifications: Valid KY driver's licenseNCI (nonviolent crisis intervention)Minimum Work Experience: Three years' experience and knowledge in the area of mental health.Prefer knowledge of crisis intervention, crisis management, and risk assessment, psychiatric diagnoses, symptomology, basic chemical dependency, age specific growth and development, and experience with a psychiatric mental health population, preferably in an inpatient setting.
