Description: Functions as a Lead of the healthcare team, coordinates departmental operations that ensure quality patient care in a safe, healing environment that is patient and family centered. Demonstrates responsibility and accountability for own professional practice. Has appropriate clinical skills to be an expert resource. Is available and supportive to employees and understands priorities/needs of the department. Is a staff role model for professional nursing practice. May recommend to management in efficient resource allocation by assuring appropriate patient assignments, staffing, and scheduling. Demonstrates performance consistent with the strategic plan of the organization and the KP nursing mission and values.

Essential Functions:

The Charge Registered Nurse demonstrates proficiency by exhibiting the following skills, competencies,&behaviors:

Leadership: Directs reviews, assigns&organizes the work of at least three (3) or more employees, of which at leastone (1) must be another Registered Nurse.Applies the KP Nursing mission, vision, values&behaviors in daily practice. Acts as a resource, preceptor&mentor to new employees, registry, students,&other team members in department specific operations&patient care activities. Collaborates with or refers to manager/clinical nurse specialist/educator any outstanding issues or concerns on the unit. Demonstrates ability to identify&resolve problems in collaboration with work team&with other departments. Ensures compliance with regulatory requirements, policies, procedures,&standards of practice. Participates in activities related to patient flow, including admission&discharge rocesses. Facilitates inter-professional/interdisciplinary communication&collaboration.

Nursing Process: Serves as a resource for clinical concerns, scope of practice&applicable state&federal regulations as well as operational concerns of providers&staff. Possesses experience&skills to be a resource for providers, RNs, LVNs, USs/CNAs, (&other ancillary staff as appropriate). Demonstrates flexibility in work schedule to meet the department dem&s of the day; facilitates&relieves staff breaks. Actively involved in assigning, orienting,&precepting&coaching staff as needed.

Documentation: Conducts audits&ensures that documentation reflects thorough assessments, incorporates physical&psychosocial findings, diagnostic tests, learning needs, barriers, multidisciplinary education plans&age appropriate care.

Clinical Outcomes: Participates/leads various aspects of department operations, staff competencies, department committees, Unit Based Teams (UBTs), new projects, quality efforts,&educational programs. Demonstrates knowledge of patients' specific conditions commonly seen in the department&care required. Interfaces with leadership, peers, physicians, other healthcare providers,&support staff regarding performance, as necessary. Actively participates, champions&motivates other staff in quality improvement in patient care, workflow,&improvement in clinical initiatives&working conditions of the department. Provides input on employees' performance evaluations through daily observation&monitoring of performance.

Workplace Safety: Adjusts staffing needs of the department to assure quality patient care, patient&staff safety, efficiency, productivity&working conditions of providers&staff. Actively models Labor-Management Partnership (LMP) Workplace Safety principles&practices.

Patient Care Experience/Customer Service: Acts as a role model for staff regarding providing a patient care experience that exceeds members' expectations. Ensures timely&appropriate management of patients. Coordinates smooth transition for patients across the care continuum. Participates in daily leadership rounds, documents rounding activities on unit rounding logs (as appropriate.). Assures hourly rounds are conducted by staff. Where appropriate, assures hourly rounds are documented according to unit/facility policy or guidelines.

Team Commitment: Ensures support of the collaborative LMP environment through unit based teams (UBTs). Identifies&supports staff professional/clinical development needs (i.e. certification, presentations, reward/recognition). Motivates staff to attain their fullest potential in improving performance, attendance,access, service&quality outcomes. Lead/facilitate daily/shift multidisciplinary huddles. Assures huddle messages are consistently communicated. Confronts difficult or conflict situations constructively&seeks appropriate assistance.

Fiscal Responsibility: Participates in resource allocation (assignments, scheduling, staffing, equipment/suppliesmanagement). Collaborates&assists with leadership, physicians,&peers in staffing, workflow&throughput to promote quality patient care, patient&staff safety, efficiency, productivity&working conditions of providers&staff.

Professional Development: Attends Charge Nurse Educational programs. Takes responsibility for professional growth&development.

Basic Qualifications:

Experience

Experienced RN's: Applicant must have a minimum of one (1) year full-time equivalent experience within the past three (3) years as an RN. This experience must be within the clinical area of specialty for the position of hire. See specialty language below.

Education

If an inexperienced RN is being considered, and appropriate KP training program is available, will consider the following: RN/IP's - New graduates and experienced RN's not meeting above requirements: Applicant must be a graduate of a BRN approved RN program and have completed 917 hours of supervised RN student clinical training or as an RN applicant, is an LVN who has graduated from an accredited, licensed vocational nursing program that included supervised clinical student experience and has a minimum of one (1) year of experience as an LVN in the clinical area of RN hire within the past three (3) years or comparable clinical area as defined by the medical facility. Applicants will be required to successfully complete a Kaiser Permanente Nursing Orientation Program. IP applicants must receive RN license within three (3) months of hire.

License, Certification, Registration

Current and valid California Board of Registered Nursing (BRN) Registered Nursing license.

BLS certification.

SPECIALTY REQUIREMENTS: ACUTE CARE

INPATIENT ADDICTION MEDICINE/CDRP - 1-year recent (within the last 3 years) full-time equivalent experience in an acute Med/Surg/Chemical Dependency setting. Working knowledge of medical detoxification/withdrawal signs and symptoms, chemical dependency signs and symptoms and/or psychiatric diagnoses and behavior interventions.

CARDIOVASCULAR INTENSIVE CARE UNIT (CVICU) (Fontana): Cardiac Surgical Unit - 1-year recent (within the last 3 years) full-time equivalent experience in a Cardiovascular ICU or completion of a Kaiser Permanente or equivalent CVICU course within past 12 months. Current ACLS card required.

CARDIOVASCULAR OPERATING ROOM (CVOR) (Fontana): Cardiac Operating Room (OR) - 1-year recent (within the last 3 years) full-time equivalent experience circulating Cardiac Surgeries or completion of a Kaiser Permanente or equivalent CVOR course within past 12 months. Current ACLS card required. Ability to scrub as required. Ability to take call within a 30 minute response time.

CRITICAL CARE - 1-year recent (within the last 3 years) full-time equivalent experience in Critical Care, or successful completion of a KP Critical Care course or approved equivalent within prior 12 months. Current ACLS card required.

DEFINITIVE OBSERVATION UNIT (DOU)/STEP DOWN UNIT (SDU) - 1-year recent (within the last 3 years) full-time equivalent experience in a Critical Care, DOU or SDU setting, or successful completion of a KP Critical Care, DOU or SDU course (as unit appropriate) or approved equivalent within prior 12 months. Basic Arrhythmia course required. Ventilator training may be required. Current ACLS card required.

EMERGENCY (ED) - 1-year recent (within the last 3 years) full-time equivalent experience in ED, or successful completion of a KP ED training program or approved equivalent within prior 12 months. Current ACLS & PALS card required.

FAMILY CENTERED CARE (FCC) - 1-year recent (within the last 3 years) full-time equivalent experience in mother/baby care or successful completion of a KP FCC training program or approved equivalent within prior 12 months. NRP card required. May be expected to cross train to at least 1 other Perinatal area.

HEMODIALYSIS (Inpatient Acute) - 1-year recent (within the last 3 years) full-time equivalent experience in acute Hemodialysis, or successful completion of a KP acute Hemodialysis course or approved equivalent within prior 12 months. Current ACLS card required. The ability to take call within a 60 minute response time

INFUSION CENTER (Acute Care) - 1-year recent (within the last 3 years) full-time equivalent experience in acute care or ambulatory care. PICC didactic course and recent (within the past 3 years) PICC insertion experience may be required. If administering chemotherapy, a current ONS Provider Card is required. Basic arrhythmia course may be required. Current ACLS card may be required.

LABOR & DELIVERY (L&D) - 1-year recent (within the last 3 years) full-time equivalent experience in L & D, or successful completion of a KP L&D course (which includes basic fetal monitoring and NRP) or approved equivalent within prior 12 months. Current NRP & ACLS card required. Current AWHONN advanced fetal monitoring required. May be expected to cross train to at least 1 other Perinatal area. Ability to scrub and circulate as required.

MED/SURG - 1-year recent (within the last 3 years) full-time equivalent experience in an acute care setting. Training or experience in chemotherapy (ONS Provider Card), neurology/neurosurgery and/or telemetry may be required.

MENTAL HEALTH CENTER - 1-year recent (within the last 3 years) full-time equivalent experience in an acute psychiatric setting. Management of Assaultive Behavior (MAB) certificate required.

NEONATAL Level of Care II (Irvine, Ontario, South Bay and Woodland Hills): 1 -year recent (within the last 3 years) full-time equivalent experience in a Level II NICU or above, or successful completion of a KP Neonatal Intensive Care Unit (NICU) course or approved equivalent within prior 12 months. Current NRP card required. May be required to train for higher level of care. May be expected to cross train to at least 1 other Perinatal area.

NEONATAL Level of Care III (all other facilities): 1-year recent (within the last 3 years) full-time equivalent experience in Level III NICU or successful completion of KP Neonatal Intensive Care Unit (NICU) course or approved equivalent within prior 12 months. Current NRP card required. May be expected to cross train to at least 1 other Perinatal area.

OPERATING ROOM (OR) - 1-year recent (within the last 3 years) full-time equivalent experience in an acute hospital Operating Room or Surgicenter, including circulating on a variety of surgical procedures in multiple specialties, or successful completion of a KP Operating Room course or approved equivalent within prior 12 months. Ability to scrub as required. Ability to take call within a 30 minute response time.

POST ANESTHESIA CARE UNIT (PACU) & PRE-OP/POST OP - 1-year recent (within the last 3 years) full-time equivalent experience in PACU or Critical Care, or successful completion of a KP PACU course or approved equivalent within prior 12 months. Current ACLS and PALS card required. The ability to take call within a 30 minute response time. If only Critical Care experience, successful completion of didactic and clinical training program will be required.

PEDIATRICS - 1-year recent (within the last 3 years) full-time equivalent experience in acute care Pediatrics or successful completion of a KP acute care Pediatric training program or approved equivalent within prior 12 months. Current PALS card required. Chemotherapy (ONS Provider card or APHON Chemotherapy & Biotherapy Provider card) may be required as specified by the department. May be expected to cross train to at least 1 other Perinatal area.

PEDIATRIC INTENSIVE CARE UNIT (PICU) - 1-year recent (within the last 3 years) full-time equivalent experience in PICU, or successful completion of a KP PICU course or approved equivalent within prior 12 months. Current PALS card required. Chemotherapy (ONS provider card or APHON Chemotherapy & Biotherapy Provider card) required within 6 months of hire.

PRE-OP/POST OP - see PACU

RN FIRST ASSIST - 3-years recent (within the past 5 years) full-time equivalent experience in the operating room, including both the scrub and circulating roles. Nationally certified operating room nurse through the Association of Operating Room Nurses (AORN) or an accredited program. Successful completion of a structured RNFA course and completion of 20 hours or 10 cases of proctoring by the sponsoring surgeon. Current ACLS card required. The ability to take call within a 30 minute response time.

SURGICENTER - 1-year recent (within the last 3 years) full-time equivalent experience in an acute hospital Operating Room or Surgicenter, including circulating on a variety of surgical procedures in multiple specialties, or successful completion of a KP Operating Room course or approved equivalent within prior 12 months. Ability to scrub as required. The ability to take call within a 30 minute response time if required.

TELEMETRY - 1-year recent (within the last 3 years) full-time equivalent experience in an acute care setting. Basic Arrhythmia course required.

TRANS TELEPHONIC MONITORING (Case Coordination Center/Downey): - 1-year recent (within the last 3 years) full-time equivalent experience in a Critical Care, DOU or SDU setting. Basic Arrhythmia course required.

HOME CARE SETTING - HOME HEALTH / PALLIATIVE CARE / HOSPICE - 1-year recent (within past three 3 years) full-time equivalent experience in a licensed Home Health, Palliative Care and/or Hospice agency as an RN/PHN. Current California Driver-s License and a clear driving record for the past two years. Proof of automobile insurance as required by law. Home Health & Hospice Trainees: Will accept a trainee with a minimum of 2 years of recent (within the past 3 years) experience in SDU, DOU, Telemetry or Med/Surg, or 4 years recent (within the past 5 years) in a Skilled Nursing facility. Current California Driver-s License and a clear driving record for the past two years. Proof of automobile insurance as required by law. Applicants will be required to successfully complete a Kaiser Permanente Nursing Orientation Program.

Preferred Qualifications:

BSN preferred.

Specialty certification preferred.
