As healthcare continues to evolve and more patients are transitioning home for treatment, we're seeking a driven, compassionate Registered Nurse to join a multidisciplinary team providing skilled care for the most acutely ill patients in the home. Key responsibilities include facilitating communication between patients and providers, developing and managing a detailed plan of care for each patient, educating the patient and his/her family, and providing skilled care for patients who are recovering from acute health events or managing chronic health conditions. This practice setting affords caregivers the opportunity to spend significant time with the patients they manage, while affording patients the opportunity to recover privately in the comfort of their own home.

This is a first shift full time Monday-Friday position. In addition, on-call and weekend rotation are required.

Education:

* Graduate from an accredited school of professional nursing.

* Bachelor of Science in Nursing (BSN) is preferred. If a diploma or Associate's Degree of Nursing, a BSN will be required within 3 years of hire date.

Certifications:

* Current state licensure as Registered (RN).

* Basic Life Support (BLS) Certification.

* Valid Ohio Driver's license (VDL)

* Must have reliable transportation and proof of automobile insurance with $100,000/$300,000 coverage.

Complexity of Work:

* Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

* Must be able to work in a stressful environment and take appropriate action.

Work Experience:

* Minimum one year clinical experience as a registered nurse in an acute medical-surgical setting; infusion therapy experience preferred.

Physical Requirements:

* Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.

* Must be able to read, write and communicate with patients.

* Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.

* Must present a professional appearance at all times and attend staff and agency meetings, as requested.

* Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.

Personal Protective Equipment:

* Follows standard precautions using personal protective equipment as required.

Responsibilities:

* Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

* Performs accurate and complete admission assessments and reassessments within specified time frame.

* Assesses patient for safety risks, abuse and neglect.

* Serves as a primary coordinator of all disciplines for well-coordinated patient care.

* Assesses and documents patients' pain using appropriate pain scale and manages patients' pain then, prioritizes and documents patient problems based on assessed needs and problems in collaboration with patient, family and health care team.

* Other duties as assigned.
