Care Manager, MSW – ED - Case Management – Celebration – Part Time

AdventHealth Celebration seeks to hire a Care Manager, MSW who will embrace our mission to extend the healing ministry of Christ.

Established in 1997 and now a 203-bed hospital, AdventHealth Celebration Health was designed as a Mediterranean resort-style facility to serve as a cornerstone of health in Disney's planned community of Celebration, Florida. Department Profile:

The Care Management objective is to create a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for services to meet individual and family comprehensive needs. Care Management is a full service inpatient department that partners closely with Physicians, clinical/nursing teams, ancillary departments and the revenue cycle teams. Care management meets the needs of our patients, families and significant others throughout the care continuum including the post-discharge needs.

Work Hours/Shifts:

Rotate days/weekends/ 10a – 10:30p

Job Summary:

Under the general supervision of the Director and/or Manager, in collaboration with the ED RN Care Manager, the ED MSW Care Manager works with the patient/family, physicians, interdisciplinary team and external agencies. The ED MSW Care Manager utilizes expertise in patient/family assessment to assist in creating a discharge plan. As needed the ED MSW Care Manager, is responsible for collaborating with the ED health care team to initiate and facilitate the discharge plan. The ED MSW Care Manager actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

  • Acute care hospital experience (Preferred)

Licensure, Certification, or Registration Required:

  • None
  • Clear/Active Licensed Clinical Social Worker (Preferred)
  • Case Management certification - ACM (Accredited Case Manager), CCM (certified Case Manager) (Preferred)

Job Responsibilities:

  • Collaborates with the interdisciplinary team in the ED for post-discharge arrangements and will work with external entities to obtain and negotiate resources to meet patient needs, as necessary
  • Interviews patients/families to complete an initial discharge assessment and to evaluate current support systems available to the patient and family, when patients are referred by the ED clinical staff.
  • Applies knowledge of community resources to the development of an appropriate discharge plan.
  • Utilizes information obtained from information systems, along with professional judgment, to help identify patients for further screening of potential discharge needs, if available and appropriate.)
  • Appropriately prioritizes cases according to assessed needs, per Care Management Department policy
  • Facilitates the discharge plan for patients with post-care needs, including but not limited to, home health, DME, ALF, or transfers to SNF.
  • Provides education to patients/families/caregivers regarding community resource options.
  • Serves as a field instructor for social work students, when needed.
