About this job
Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. A certain degree of creativity and latitude is required. Reports to the Case Management Director.Must be a registered nurse in TN with 2-4 years of clinical and case management experience. This is a PRN position that may develop into consistent hours.
