Oncology Pharmacist Regional (Willoughby Hills)

Job Summary:

The Clinical Pharmacist is responsible for pharmacy functions inside the Willoughby Hills Oncology Infusion clinic. This includes but not limited to: reviewing chemotherapy orders, checking compounded product, education of physician/nursing/technicians, patient counseling, oral chemotherapy counseling and precepting of students/residents.

Ensures the appropriate selection, dosing, administration, and dispensing of medication orders. Manages the oversight and supervision of activities of pharmacy technicians.

Responsibilities:

* Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.

* Recommends formulary alternatives when appropriate.

* Compounds and dispenses medications for delivery to patient care areas.

* Provides drug information and education to medical staff.

* Communicates with physicians and other clinicians as necessary to facilitate care.

* Other duties as assigned.

Education:

* PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.

Certifications:

* Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire.

Complexity of Work:

* Requires critical thinking skills, decisive judgment and the ability to work unsupervised or with minimal supervision.

* Must be able to work in a stressful environment and take appropriate action.

Work Experience:

* Internal candidates: Minimum of six months as a Pharmacist II

* External candidates: Minimum of two years experience in hospital pharmacy OR have successfully completed a PGY1 Residency.

Physical Requirements:

* Typical physical demands include ability to perform functions of the job while reaching and stretching, and manual dexterity sufficient to perform computer functions of job.
