Registered Nurse - Full-time / Part-time

Sterling Care is a growing and innovative high quality certified home health agency located in Greenwich, Connecticut. We are seeking dedicated and accomplished full-time Registered Nurse Case Managers with a minimum of two years of recent experience with a Certified Home Health Agency experience to service patients in lower Fairfield County, Connecticut. As a Registered Nurse Case Manager, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

You will be responsible for:

- Making home visits to clients in designated areas.

- Performing assigned nursing duties, including wound care, treatments, and procedures.

- Monitoring patients' conditions.

- Continually assessing and revising the nursing care plan, and participate in the multidisciplinary care plan.

- Following up with, executing, and properly documenting doctors' orders.

- Performing client assessments as necessary.

- Case management and coordination.

- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touchpad tablet.

Qualifications include:

- A current license as a Registered Nurse with a minimum of two years of recent experience with a Certified Home Health Agency. 

- Demonstrated ability to read, write, and effectively communicate in English.

- Ability to work independently and manage time efficiently.

- Strong Interpersonal skills.

- Excellent computer skills.

- Prior experience with electronic medical records (EMR).

- Ability to travel to cases as assigned.
