Summary:
To develop a leadership role with restorative care in the Pathways Restorative Program by working directly with residents and coworkers to maximize each resident’s optimal level of functioning.

Requirements:

  • Promotes resident independence by focusing on the residents’ abilities.
  • Provides encouragement and cuing for independence during activities of daily living (ADL’s), including but not limited to bathing, dressing, grooming, etc.
  • Coordinates with therapy, PCA’s, nursing, and the Fitness Coordinator. Ensures consistent follow through with recommendations regarding specific resident care.
  • Guides residents in the use of assistive devices such as walkers, canes, wheelchairs, and other adaptive equipment.
  • Takes the initiative and guides other co-workers to encourage residents to participate in exercise, ambulation, range of motion, therapeutic activities, etc.
  • Interacts and communicates with residents and their families in a manner that shows respect and promotes confidence in the co-workers. Refers them to the nurse or administration for service/care concerns.
  • Participates in the planning of customer service needs using the process implemented within the facility.
  • Participates in other tasks to assist with resident care throughout the day such as laundry, dining services, etc.
  • Communicates consistently with the shift leader to provide quality care for the residents.
  • Practices good safety awareness and procedures. Proactively addresses safety concerns and brings them to the attention of appropriate personnel.
  • Collaborates with the Community Life Department to provide therapeutic activities and promote social programs.
  • Encourages residents to plan, attend, and participate in social functions in and outside the facility.
  • Recognizes and respects residents’ rights.
  • Practices responsible team work by participating in facility meetings, contributing to work group efforts to improve operations, and a willingness to lend assistance where needed.
  • Works every other weekend and holiday.
  • Other duties as assigned.

  • Communication skills that include careful listening, sensitivity to emotions attached to the message, and thoughtful responses.

  • Displays personal initiative to complete work with minimal supervision and address problems with the proper individuals.
  • Reliable work attendance.
  • Commitment to confidentiality for both resident and co-worker information.
  • Discloses personal information only to appropriate supervisory co-workers.
  • Approaches work with a flexible and positive attitude. Ability to change the course of activity with changing demands without experiencing undue stress or frustration.

Work Environment:

  • Requires frequent standing, walking, bending, pushing, kneeling, stretching, and lifting a minimum of 25 lbs.
  • Be able to push/pull 100 lbs.
  • A gait belt is provided and to be on you at all times during your working hours.
  • Requires normal range of hearing and vision, hand/eye coordination, and manual dexterity.
  • Requires knowledge or office equipment (i.e. fax, telephone, intercom system, computer, printer, fire board panel, television, VCR/DVD, small appliances, etc.)
  • Workplace includes the entire building, surrounding grounds, swimming pools, and all areas necessitated by off-site field trips and events.

Minimum Qualifications:

  • Experience – Prior personal care experience and/or rehab experience.
  • Education – Minimum High School Diploma or GED
  • Certification/License – Valid driver’s license with no major violations within the past five years.
