Medical Officer Admin (Clinical Director) Sells Hospital Position Summary: Incumbent serves as Director of the Division of Clinical Services (Clinical Director) at TONHC, and has the full responsibility for overall administration and direction of medical and ancillary staff. Responsibility includes identifying needs and coordinating a variety of services and activities within TONHC and in the community to promote optimal health services. TONHC provides medical and support staff services for an inpatient unit, ambulatory clinics, and emergency medicine. In addition to the above, the assignment includes providing medical care services. Assignments involve the full range of problems found in the medical field, including the very difficult, where there is responsibility for recognizing difficult-to-identify symptoms or signs, and with responsibility for developing a full treatment regimen involving knowledge of state-of-the-art techniques. Incumbent also has the responsibility for medical students, interns, and residents assigned for training. Advises Medical Officers on procedures to be followed in the care and treatment of difficult cases. This position requires education and licensure as an M.D. or D.O., three years of postgraduate residency training or equivalent, certification by (or eligibility through) a member board of ABMS (American Board of Medical Specialties), and at least five years of clinical experience, including one year of leadership experience in an administrative role. Scope of Work: This position is located in the Division of Clinical Services, Tohono O'odham Nation Health Care (TONHC), Sells, Arizona, approximately 60 miles west of Tucson, Arizona. The position administers, coordinates, and directs all health services for TONHC. TONHC provides inpatient and outpatient medical and dental care to eligible Native American and Alaska Native persons residing in or near the Tohono O'odham Nation. TONHC is the primary source of health care for the approximately 24,000 local members of the Tohono O'odham Nation. TONHC consists of TON Sells Hospital, a ten bed hospital accredited by The Joint Commission (TJC) and located in Sells, Arizona; three Health Centers: TON San Xavier Health Center, Tucson, Arizona; TON Santa Rosa Health Center, Santa Rosa, Arizona; and TON San Simon Health Center, San Simon, Arizona; and a Community Health Program. TONHC performs approximately 200,000 outpatient visits annually. The TONHC Community Health Program consists of School Health Program, Public Health Nutritionist, Environmental Health Specialist, and Health Promotion Disease Prevention Coordinator. The Preventive & Public Health Medical Officer, who supervises the Community Health Program, is supervised by the Clinical Director. The Clinical Director (CD) directs the multi-specialty Medical Staff, along with Dental and Podiatry services. The CD additionally supervises (1) the Deputy Clinical Director (who supervises the Supervisor of Laboratory Services, the Supervisor of Social Work Services, the Supervisor of EMS, and the Supervisor of the Eye Program); (2) the Preventive & Public Health Medical Officer (who supervises the School Health Program, Public Health Nutritionist, Environmental Health Specialist, and Health Promotion Disease Prevention Coordinator); and (3) the Director of Professional Services (who supervises Diabetes Prevention & Coordination, Medical Imaging, Nutrition & Dietetics, Pharmacy, and Physical Therapy). The overall emphasis of this position is to provide for the planning, development, organization, integration, administration, and evaluation of all Clinical Services within TONHC. As a physician, the CD also performs and provides diagnosis, care and treatment of inpatients, ambulatory care patients and emergency patients for a full range of clinical scenarios. As an expert licensed provider, the incumbent is responsible for direct and indirect specialized child, adolescent, and adult services, and for Continuing Medical Education (CME). Essential Duties and Responsibilities: Clinical Duties (30%) Incumbent is responsible for maintaining the highest standards of professional medical services for TONHC. Activities in support of this goal include the following: Provides direct patient care in the medical specialty in which the incumbent is Board certified/eligible. Administers, coordinates, and directs all health services for TONHC, including inpatient and outpatient services. Plans and conducts regular and specialty clinics, as appropriate to local healthcare needs. Assures the availability of a well-qualified Medical Staff. Provides personal consultation with members of the Clinical Staff on matters of patient care management. Plans and conducts regular patient care management conferences, employing the assistance of other medical specialists as appropriate. Monitors quality of medical care and health records maintenance through programs like Ongoing Professional Practice Evaluation (OPPE) and Health Information Management Record Reviews, and through direct participation in and observation of patient care practices. Provides mechanisms to ensure a uniform standard of quality patient care throughout all TONHC clinical sites. Implements Clinical Programs that improve delivery and quality of patient care, e.g., Government Performance Results Act (GPRA) Clinical Objectives, Electronic Health Record Systems, Telemedicine Programs, Disease Management Guidelines. Advises and assists the TONHC CEO (Chief Executive Officer) in creating and maintaining effective organization and management of TONHC so as to offer responsive, high quality healthcare. Plans for expansion, revision, or contraction of services to meet the changing needs of the local community. Promotes integration of Clinical Services Division activities with those of other TONHC Divisions, TON DHHS (Department of Health and Human Services), and other local, state, and federal programs. Performs and provides care to patients primarily in ambulatory clinic and inpatient settings; provides patient care and consultation in the Emergency Department as needed. Routine duties, even if rarely required, shall include providing medical care and public health services to individuals eligible for services provided by TONHC in homes, schools, clinics, job sites, and other community locations within the TONHC service area.  Delivers infants when necessitated by urgent/emergent circumstances, and provides or arranges for postpartum care. Ensures preparation of appropriate medical records for all patients seen to ensure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Coordinates and integrates information obtained into ongoing health care information systems utilized by TONHC. Disseminates clinical reports and correspondence concerning patients to physicians, hospitals, and other individuals and facilities. Works closely with allied health professionals to provide effective interdisciplinary health care services and treatment. Communicates effectively with patients and families in situations of medical, psychosocial, and cultural complexity. Program Support Duties (30%) Works on a day-to-day basis with the CEO, Administrative Officer, Director of Nursing, Director of Professional Services, and other Executive Staff, with responsibility for fully participating in top management discussions, decisions, and policy making; shares full accountability in management actions. Such work involves the following kinds of tasks: Serves as member of TONHC Executive Committee. Integrates the functions of the Clinical Services Division into the primary functions of TONHC, and coordinates interdepartmental and intradepartmental services. Chairs the Executive Committee of the Medical Staff. Makes committee member recommendations to CEO for all Medical Staff Committees. Represents Medical Staff concerns and viewpoints to other TONHC Divisions, TONHC Executive Committee, and TONHC Governing Body. Develops short-range and strategic plans and objectives for the Division that support the mission and strategic objectives of TONHC. Develops and implements Division policies and procedures that guide and support the provision of services and promote progress toward the goals and objectives. Continually assesses and improves the quality of care and services provided by the Division. Ensures that quality control programs are maintained within the Division. Ensures that corrective actions are taken when opportunities for improvements are found. Recommends to the CEO off-site sources for needed Clinical Services when TONHC does not provide such services. Ensures that the Service Unit is organized and functions in a manner commensurate with CMS Conditions of Participation (Medicare/Medicaid Standards) and The Joint Commission (TJC) standards on a continuous basis. Ensures the maintenance of effective Quality Improvement, Risk Management, and Compliance programs. Maintains TONHC's TJC accreditation. Enlists support and participation of the Medical Staff in Ongoing Professional Practice Evaluation (OPPE). Ensures that actions are taken when opportunities for improvement are discovered, and that the effectiveness of those actions is evaluated and communicated to the Quality Improvement Committee and Executive Committee. Ensures that the results of OPPE are used in the review and re-appointment process for the Medical Staff. Ensures continuous surveillance of the professional performance of all individuals who have delineated clinical privileges; maintains accurate and current credentialing and privileging information on members of the Medical Staff; recommends clinical privileges for each member of the Medical Staff. Ensures appropriate medical oversight of the Clinical Laboratory, Medical Imaging (Diagnostic Radiology), and Emergency Medical Services (EMS). Represents the Medical Staff on Safety, Infection Control, Quality Improvement, Managed Care, and other TONHC Committees, or appoints other qualified representatives. Supervisory Functions (30%) Exercises overall supervisory responsibility and authority for the activities of the Clinical Services Division: Medical Staff, including Dental, Optometry, and Podiatry services; Professional Services Program, comprising Diabetes Prevention & Self-Management Education, Laboratory, Medical Imaging, Nutrition & Dietetics, Pharmacy, Physical Therapy, Pre-hospital Emergency Medical Services, and Social Work Services. Provides leadership and direction in human resources management within the Clinical Services Division. Provides both Clinical and Administrative supervision of subordinate managers and supervisors. Delegates authority to and provides general program direction to subordinate managers and supervisors in various administrative matters. Assists them in identifying existing and potential problem areas and in developing sound solutions thereof. Advises them in scheduled functional reviews of their services and applies performance standards to measure the extent to which their activities are achieving acceptable goals. Establishes, reviews, and evaluates the performance standards of subordinate managers/supervisors, and provides appropriate feedback based on their performance reviews. Collaborates with CEO in formulating, supervising, and integrating Clinical and Administrative functions. Knowledge, Skills, and Abilities: Program Scope and Effect The ultimate purpose of this position is for TONHC to offer the highest quality health care in its role as primary care and referral center for the Tohono O'odham Nation. The work has a major impact on strategic planning for the safety, quality of care, quality of patient care data, and economic efficiency of care provided by TONHC directly and through its contract providers. The annual operating budget of the Division of Clinical Services, under the direct supervision of the incumbent, is estimated at 20 million dollars. Duties involve establishing policy criteria, assessing program effectiveness, investigating and analyzing a variety of conditions, problems, or questions which affect a wide range of activities, and stimulating responsive action toward the achievement of quality care. The incumbent makes recommendations for the expenditure of third party revenues to maintain and improve TONHC performance. Effective management of the Clinical Program is critical to the Tohono O'odham Nation, as TONHC is the only local health care system. Hospital accreditation depends heavily on the incumbent's performance. 0rganizational Setting Incumbent works under the administrative direction of the CEO, who establishes work assignments in terms of broadly defined missions or functions. The employee has the responsibility for planning, designing and carrying out programs, projects, studies or other work independently. The incumbent's work crosses all organizational lines throughout TONHC. Results of the work are considered as technically authoritative and are accepted without significant change. Recommendations for new projects and alteration of objectives are evaluated for such considerations as availability of funds and other resources, broad program goals or priorities. Supervisory and Managerial Authority Exercised Work Planning: Formulates and implements all administrative programs and plans to satisfy varying requirements and priorities for services and other TONHC objectives. Collaborates with Division Managers and Branch Chiefs to negotiate, decide on and/or coordinate working relationships. Oversees quality of patient care, treatment, and services throughout TONHC. Provides oversight of the process of analyzing and improving patient satisfaction and patient safety. Oversees delivery of safe quality care by participants in professional graduate education programs by assuring supervision by a licensed independent practitioner with appropriate Clinical privileges for each member in the program carrying out patient care responsibilities. Provides leadership in at least the following activities: 1. Education of patients 2. Coordination of care, treatment and services, as relevant to patients. 3. Oversight of accurate, timely and legible completion of patient's medical records. 4. Oversight of the assessment processes for licensed independent practitioners and use of assessment information in the ongoing evaluation of practitioner's competence. Provides leadership in the evaluation of competency and scope of privileges for practitioners. Oversees licensed independent practitioner's participation in continuing education to maintain clinical skills and competency. Work Direction: Conducts meetings and conferences with subordinate supervisors and staff to explain work requirements, methods and procedures, giving special instructions on difficult activities, and answering technical questions about work assignments. Formulates and issues written instructions and procedures for non-routine or complex assignments, new or changed programs, and functions and/or processes. Reviews, approves, modifies, or rejects changes in functions, procedures, personnel assignments, and other operating matters proposed by subordinate supervisors. Makes decisions on problems not covered by precedents, protocols, or established policies. Revises and analyzes records, reports, and other evidence of work performance in order to evaluate subordinates' performance. Reports to TONHC Executive Committee on progress in resolution of problems and achievement of objectives. Continually studies operations to make improvements and recommendations in the quality and quantity of services provided. Personnel Administration: Directly supervises and/or supports supervisors of 200+ employees who engage in the performance of a variety of direct patient care and support activities. Exercises approval or disapproval authority for personnel actions requested by subordinate supervisors. Ensures subordinate supervisors implement and adhere to internal programs and plans such as the IHS Merit Promotion Plan, Excepted Service Examining Plan, position management responsibilities, career development plans, performance appraisal system, counseling services, and other functions to achieve equitable treatment of employees. Hears group grievances and serious individual employee complaints. Familiar with and abides by the Labor-Management Bargaining Contract. Reviews and decides penalties for disciplinary and adverse action cases initiated by subordinate supervisors, as appropriate, and has authority to issue same to his/her subordinates. Approves, modifies or rejects career development plans, training requests, employee utilization proposals and similar matters. Estimates and justifies human and financial resources needed to accomplish program objectives and recommends approval to high-level management. Approves, modifies or rejects formal requests for promotions, reassignments, status changes, awards, selections, and other types of personnel actions. Establishes, monitors, conducts progress reviews, and final performance appraisals of subordinate supervisors and other key personnel of the Division. Serves as the reviewing official for all evaluations prepared by subordinate supervisors.
