Saratoga Hospital Medical Group - Primary Care Wilton (Saratoga Family Physicians) This practice is located on our Eastern campus in Saratoga Springs. This is a large primary care practice with eight family physicians, one internal medicine and three advanced practitioners. Excellent support in adjoining Wilton Medical Arts that offers advanced imaging, lab, and urgent care, with our PT/OT in the neighborhood. In addition, specialist offices of otolaryngology, interventional pain management and ophthalmology are located across the parking lot. Saratoga Hospital Medical Group - Primary Care Schuylerville (Schuylerville Family Health) This is a great opportunity in a small town setting with a rural component, 10 miles from Saratoga Springs. A physician who enjoys a good mix of adult medicine and pediatrics will love this intergenerational practice located on the edge of the New York State Canal System in a region steeped in Saratoga's history. The practice is nicely situated across from the Schuylerville Central School. The practice offers a laboratory draw station on site. The site is well-managed with excellent support staff. The physician will work with an experienced physician and advanced practitioners. Saratoga Hospital Medical Group - Primary Care (Saratoga Community Health Center) Saratoga Community Health Center offers a holistic approach to care with an integrated health care delivery model that includes primary care, behavioral health and addiction medicine, as well an on-site dental center. Two family medicine physicians and an advanced practitioner look forward to an enthusiastic third physician joining the team. The payer mix varies from private payer to a large population of Medicaid and Medicare. This is a mission-focused practice caring for the insured and underinsured population of our city. The practice is in the process of becoming a Patient Centered Medical Home designated through NCQA. The practice focuses on patient-centered care, with enhanced quality and patient access. Our practice team includes on-site care managers and referral coordinators. Saratoga Community Health Center is located in the heart of Saratoga Springs, a walk to our downtown shops and cafes! Saratoga Hospital Medical Group - Women's Primary Care Practice 100 % outpatient Women's Primary Care serving patients adolescents and adults, from age 13 to geriatrics, with a family medicine physician and family nurse practitioner. Our practice is located adjacent our Saratoga Hospital Medical Group: Midwifery practice, with six certified nurse midwives and Saratoga Hospital Medical Group: Endocrinology & Diabetes. The practice is located in a new, mixed-use residential and commercial space at Park Place at Wilton. Services include: Primary care, including sick visits, routine physicals, and preventive care; Diagnosis, treatment, and management of acute and chronic illness including diabetes, high blood pressure, and osteoporosis; breast exams; peri-menopause/menopause care; family planning services; patient education; immunizations. Saratoga Hospital Medical Group - Primary Care Scotia - Glenville (Scotia Glenville Family Medicine) This practice is comprised of six family medicine physicians and three physician assistants. This practice offers the Patient-Centered Medical Home model. The practice is certified by the NCQA (National Committee for Quality Assurance) as a Level 3 PCMH. In addition, all of our practices are participating in a Comprehensive Primary Care initiative with the Centers for Medicare & Medicaid Services. Saratoga Hospital Medical Group - Primary Care Malta This is an extension of our Scotia Glenville Family Medicine (SGFM) practice located approximately 9 minutes southwest of Malta. Family Medicine Physicians would fit this traditional family practice. Currently three part time family medicine physicians and a PA enjoy outpatient family medicine at this location. Physicians from SGFM rotate through Malta. Malta MDs will also rotate through SGFM and share call in this group. The practice is located above Malta Med Emergent Care, which is open 24 hours for emergent care, and offers advanced imaging and lab. Mammography and DEXA scanning, an infusion center and several specialists are located adjacent to the practice. Saratoga Hospital – Primary Care Mechanicville This is Saratoga Hospital's newest primary care practice. This intergenerational practice, established in 1954, by Stephen Sgambati, Sr., MD, has served residents of the community and surrounding area for more than 60 years. The practice offers primary care, sick care, minor surgical procedures, lab and more. Specialists and services of Saratoga Hospital are located in neighboring Malta & Wilton. Saratoga Hospital Medical Group - Inpatient Medicine (Saratoga Inpatient Physicians) The practice seeks friendly, team-oriented, caring physicians with a strong work ethic and a sense of humor to join their 14-member, cohesive group, in one of the most established programs in the Capital - Saratoga Region and beyond. The group has two full time nocturnists and a growing staff of mid-level providers, two who provide daily coverage in 12-hour shifts. Our physicians work 14 days per month. Exceptional skills and interest in the care of inpatients would be a good match for this growing practice. Excellent communication and education skills with patients, caregivers and staff are important criteria for the success of this program. Urgent Care at Malta Med Emergent Care Family Medicine Physicians are welcomed to apply for a new position in a state-of-the-art enhanced 24-bed urgent care center, open 24/7, with 36,000 annual visits. Work with advanced practitioners, with scribes, and enjoy strong nursing support, full ancillary services including laboratory, advanced imaging including x-ray, CT and ultrasound. Physicians with experience working or moonlighting in emergency departments would be a great fit for this practice. A highly competitive compensation package will be offered.
