Case Manager

This position is part of a multidisciplinary team. The case manager delivers services to clients who have been referred to our agency for mental health, housing and chemical dependency issues.  These clients could have a multitude of agencies involved in their treatment that will require intensive, intentional, collaborative treatment within the guidelines set forth by the Ohio Department of Mental Health and Addiction Services. The position assures compliance of standards in the area of evaluation, assessment, treatment, and service delivery to clients. The case manager will coordinate services with the counselor and all referring agencies. The Primary duties include; knowledge and utilization of local community resources, quality case management assessments, timely documentation and case management services to all program referrals. Working with a vast array of mental health clients to include but limited to: homeless, housing referrals, severely mentally ill, substance abuse (MAT) clients and students who need assistance with resource implementation.   Possesses working knowledge of all phases of the job and the various techniques and skills necessary for efficient completion of tasks, including knowledge of human development and the social environment, assessment, treatment planning, treatment, etc.    Education: Associate's Degree or 3 minimum of years' experience. Experience: Preferred experience in providing Mental Health Case Management.
