This is a professional nursing position providing enhanced school health services to assigned schools and the students while school is in session, according to the Supplemental School Health Program. The incumbent of this position must come to work and exhibit an attitude of friendliness and eager customer service towards all clients and act respectfully and courteously to fellow employees and clients. The incumbent in this position is responsible for the integrity and confidentiality of data sets under his/her control. The incumbent needs to possess specific School Health Nursing knowledge and works under indirect supervision.

SERVICE:

A. Provides nursing assessments and mandated screenings to identify existing or potential health problems. Any health problems are referred for appropriate follow-up, including home visits as required.

B. Provides nursing services to clients as indicated. These include, but are not limited to immunizations, medications, treatments, laboratory procedures and emergency first aid as needed.

C. Responsible for on site guidance of those providing technical health support activities including, but not limited to, medication administration, complex medical procedures, and carbohydrate counting.

D. Maintains updated pertinent health data for student records; reviews health records for identification of health problems which may be preventable, communicable or treatable and may interfere with growth and development or the educational achievement of the student. Completes and annually reviews Individual Health Care Plan as needed.

E. When appropriate, works in health department clinics.

COUNSELING AND EDUCATION: Provides individual counseling for a variety of health topics, including but not limited to pregnancy, sexual abuse, substance abuse, growth and development, physical activity and general health.

PLANNING AND COORDINATION:

A Participates as an active member on school committees that are related to the health of the student and school.

B. Acts as Consultant for screening process required by the School Health Plan. Responsible for rescreens and activities related to follow up process.

C. Conferences and provides case management of clients with appropriate groups or individuals.

D. Plans all activities needed to fulfill the requirements of the Supplemental School Health Program.

EVALUATION:

A. Maintains all required data base information as listed in Plan.

B. Maintains student health records and immunization records.

C. Participates in Quality Improvement and Reviews.

D. Completes EARS, HMS leave, Go Travel as required. Fulfills requirements for timeliness of data submission and annual report data collection.

OTHER DUTIES:

A. Participates in staff meetings, staff development and continuing education programs.

B. Assists other school other school health nurses as needed.

C. Performs other duties as required. This position has the "Need to Know" for the following information sets:

Medical records, school health records, nursing logs, medications, lab, X-rays and computer generated/listings.
