LPN Case Manager - Full-time / Part-time

Looking for a LPN, licensed in the state of Florida, for a specialty pharmacy in Orlando Florida.

This individual must be able to work a 10 hour shift from 9:00am - 8:00pm. These shifts would be Monday - Friday with either Wednesday or Thursday off. This person would handle about 40 outbound calls a day. Review patient information and obtain missing clinical documentation, provide clinical support and administrative assistance.

Main responsibilities;

• Review patient information and obtain missing clinical documentation from physician offices and insurance companies.
• Interpret clinical information such as laboratory results and diagnosis codes represented in patient medical records.
• Enter clinical information in internal and external databases.
• Document all communication efforts with patient, physician and insurance company and relevant information into internal and external systems.
• Verify insurance and benefits information for highly complex disease states in order to schedule medication delivery.
• Conduct monthly counseling to patients and their family to ensure adherence to provider's therapy plan.
• Assist with insurance verification and patient care coordination.
