Position Summary

OhioHealth Group’s (OHG) Case Manager is responsible for promoting continuity of care through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates care options and services available to patients through Health4 (the clinically integrated network and product from OHG), their benefit plan, and the community that meet the individuals’ health care needs.

Patients interacting with OHG’s Case Managers may fall into one or more of the following categories:

Targeted chronic conditions requiring complex case management

Accessing care in the emergency department

Transition from one care setting to another

Ambulatory episodes of care.

By proactively identifying patients, contacting them and actively coordinating their care, OHG’s Case Managers promote value-based outcomes (quality, access, and cost) for the patients they serve.


Minimum Qualifications

MINIMUM QUALIFICATIONS
Education, Certification, Registration, LicenseBachelor's Degree
Field of Study: NursingYears of Experience: 2

SPECIALIZED KNOWLEDGE Registered Nurse with current, unrestricted license in the state of Ohio. Bachelor's degree in nursing or healthcare related field preferred. Minimum 3 years clinical nursing experience. Familiarity coordinating care options and services available to patients. Current clinical knowledge. Familiar with concepts of motivational interviewing. Demonstrated organizational skills with the ability to prioritize tasks. Ability to apply independent judgment and good problem solving skills. Computer proficiency. strong knowledge of case management principles. Knowledge of community resources. strong verbal and written communication. Prior case management experience in a health care organization preferred. Previous home care or managed care experience. Certified Case Manager (CCM) or willing to pursue within 2 years of hire.
