Summary\: The Cardiac Valve Navigators primary function is guiding cardiac valve patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Cardiac Valve Navigator's role additionally includes providing education and emotional support for patients as well as their family members, serving as a liaison between the patient and various healthcare providers. The Cardiac Valve Navigator partners with the physician leaders of cardiology services and other related service lines and clinical program leaders to ensure development of a comprehensive, market competitive, and leading edge clinical program.

Essential Functions\:

1. Reviews initial patient referrals to ensure appropriate testing per Valve standing orders, national practice guidelines and quality standards and communicates with patient throughout plan of care.

2. Provides coordination of care for the patients receiving pre-appointment treatment including review of the patients referred diagnosis, medical history and physician disposition.

3. Working with the valve & structural heart disease physicians, makes adjustments to schedule as seen necessary throughout the day and follows up with diagnostic testing & procedures as to the progress.

4. Functions as a clinical liaison and resource for the hospital program including serving as a liaison between physician/physicians offices. Facilitate interaction and communication with health care staff and providers.

5. Works in collaboration with the Valve team to ensure appropriate compliance with Transcatheter Valve Therapies (TVT) Registry, a condition for payment from Medicare.

6. Plans and coordinates the initiation of research study protocol, and the establishment of operating policies and procedures.

7. Ensures the smooth and efficient day-to-day operation of research and data collection activities; acts as the primary administrative point of contact for internal research staff and as the principle operational liaison for other research organizations, funding agencies and regulating bodies.

8. Develops and maintains data base for tracking outcomes and initiates performance improvement processes to correct variances.

9. Accurately documents nurse navigation activities on each patient. Tracks interventions and manages outcomes including patient's functional outcomes, provider satisfaction, and appropriate resource utilization.

10. Coordinates and executes other responsibilities as needed. Assists in development, documentation, and analysis of program performance. Assists in the collection and analysis of information to measure the value of the program.

Qualification:

Education\: Associate's degree (ADN) or Diploma in Nursing Program required. BSN preferred.

Experience\: Three (3) years experience with clinical specialty patient population required. ICU & Case Management experience preferred.

Skills\: Compile statistics; computer literacy, data entry, retrieval, reporting; input data into computer programs; interview others; research information; strong analytical and organizational skills; and team facilitation and meeting skills. Excellent human relations and oral/written communications skills. Computer skills including word, excel, and graphic. Spanish speaking preferred.

Licenses/Certifications\: "Required\: Current Arizona RN license or compact state license and American Heart Association (AHA) BLS.

Preferred\: National clinical specialty certification."
