Job Purpose:

This position involves nursing in LIFE participant homes primarily, but also may include working in the LIFE clinic/center, and nursing home visits. As part of an interdisciplinary team (IDT), assesses and monitors participants' health status in the home, provides nursing care, administers prescribed medications, educates and counsels participants and families in collaboration with primary care physician and other team members. Conducts initial assessments and periodic reassessments, develops a plan of care, regularly informing the IDT of the medical, functional, and psychosocial condition of each participant in his/her home environment, and remaining alert to pertinent input from other team members. Educates and supports participants' caregivers in the disease process and care giving techniques. Supervises personal care assistants in the home. Documents changes in a participant's medical record and adheres to all established documentation policies and procedures.

Major Duties and Responsibilities:

1.Performs initial nursing assessments of participants' in their home environment to determine their individual need for skilled nursing and/or assistance with activities of daily living and coordinates plan of care with caregivers. Recommends, with input from occupational therapy, nursing and other team members, a care plan for each participant who needs home care.

2.Develops a nursing care plan, setting long and short-term goals to meet preventive, restorative, maintenance, and palliative care needs. Also assesses functional status and personal care needs, and develops personal care plan.

3.Serves as a member of the IDT and participates in initial assessments, care planning, and ongoing re-evaluations of participant care, changing the plan of care as the participant's needs change.

4.Assesses and triages participants in person and on the phone for care in the clinic, and facilitates efficient, effective patient care in the home.

5.Responds to emergencies, assisting the participant, primary care provider, staff and family. Assists with transitions and coordination of care across settings, i.e., hospital, nursing home, LIFE center and home.

6.Supervises clinical aspects of home health aide care in participant's homes. Coordinates with In-Home Services Manager.

7.Indicates need for dietary assessment and refers to dietitian as needed.

8.Administers medications and treatments ordered by the primary care physician (or nurse practitioner). Develops and implements an individualized "home care plan" and education program for participant's care giver (s) as appropriate.

9.Assists with management of the pharmacy dispensary, following all policy and procedures relating to control and storage of narcotics, and emergency medicines and equipment.

10.Provides health education to participants and family members, staff, and volunteers as necessary.

11.Participates as a personal caregiver.

12.Participates in the activities of the Center as needed.

13.Provides evening and weekend on-call phone coverage on a rotating basis to triage, troubleshoot, and coordinate participant care with the physician on call.

14.Maintains a clean, safe, care environment, promotes infection control, accident prevention, and reports potential hazards.

15.Demonstrates professional practice in patient care by use of established standards in planning, organizing, implementing, and evaluating.

16.Maintains current, written and electronic records in compliance with regulatory standards; prepares reports and correspondence.

17.Develops and implements participant bowel and bladder training programs.

18.Demonstrates respect for participants' homes and belongings and asks permission and explains suggested changes in the home environment related to care of the participant.

19.Demonstrates understanding of participants as unique individuals in need of loving, supportive, emotional, physical and spiritual care.

20.Demonstrates positive interpersonal relations in dealing with fellow employees, supervisors, and other department employees to maximize positive employee relations and productivity.

21.Supervises, coordinates, and evaluates students as appropriate.

22.Performs duties in accordance with ANA Code for Professional Nurses.

23. Administers competency testing to appropriate PACE staff.

24.Other duties as assigned.

Minimum Qualifications:

Education:

Graduate of a NLN or CCNE Accredited RN Program.

Current CPR certification.

Licensure: Current RN licensure in the State of Delaware.

Experience:

One year acute care experience with an adult population and

One year experience as a community health or long term care nurse or equivalent experience.

Minimum of one year experience in working with the frail or elderly population

Possess:

Demonstrated ability to work in an interdisciplinary team setting preferably with frail or chronically ill elderly persons.

Driver's license in good standing and availability of a car for daily home visits.

Medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Age Specific Criteria:

Demonstrates skills and knowledge necessary to assure assessment, treatment or care is appropriate to the age of the patients served. Care is provided for all patients including;

Contacts within the organization involve all levels of individuals and in various situations that include one on one contact, team dialogue, meetings, and committees. No special requirements exist to communicate in languages other than English.

Environmental Conditions:

Working conditions are both indoors at the adult day center and also at participants' homes in the community. Normal health and accident hazards found in a health care setting and when visiting homes in the community are encountered on a daily basis. Exposure to blood, body fluids, and other infectious hazards.

Equipment Used:

Devices associated with a medical clinic work environment can be used on a daily basis to include electrical and non-electrical equipment.
