* Level 2 Background screening on file with AHCA
* CNA License or HHA training certificate (minimum of 40 hours training from an ACCREDITED training agency is required)
* Current CPR card (Certified by American Red Cross or American Heart Association or Health and Safety Institute)
* Proof of Alzheimer's training (2 Hours)
* Proof of HIV training
* Proof of Elder Abuse training (NOT Domestic Violence Must say Elder Abuse )
* Proof of Assistance with Self-Administration of Medication training (2 Hours)
* Valid driver s license
* Proof of current auto insurance
* Social Security Card
* Statement of Health from a physician, dated within the last six months, that states you are \"Free from communicable disease\"
Companion Caregivers must have:
* Level 2 Background screening on file with AHCA
*HIV/AIDS Training Certificate
* Proof of Alzheimer's training (2 Hours)
