Requires assistance with grooming, personal hygiene, toileting, medication reminders (has history of pocketing and/or not swallowing medications), meal preparation, and housekeeping.

Diagnosed withdiabetes, anemia, BPH (enlarged prostate), HTN, anemia, and dysphagia (difficulty swallowing). Functional abilities are limited by hemiplegia and visual impairment. Needs standby/hands on assistance daily to transfer to tub. Uses an electric wheelchair for mobility.
Has condom catheter that he uses at night (is unable to apply); uses urinal; wears pull-ups. Has rare episodes of incontinence. Poor vision. Requires hands on assistance with bathing, dressing, grooming, hair care, nail care, housekeeping, meal preparation and medication reminders.

Requires total care with all activities of daily living; when in bed is to be turned every two hours. These turns are to be charted. Needs incontinence care; wears briefs. Is unable to grasp; has no hand strength in either hand; must be fed every meal.

Client has difficulty rationalizing and reasoning at times but is alert and oriented. Suffers form short and long term memory loss. Requires assistance with bathing, dressing, meals. Needs standby and assist for safety with eating. Needs standby assist for transfers; he is able to self propel in wheelchair. Requires standby by and assist with walking and uses a walker. He is continent of bowel and bladder and requires assist with self cleansing. Client does smoke outside the apartment.

82 year-old male client. Retired veteran needs assistance with light housekeeping. He would like like the caregiver to do his floors every other week with Murphy's oil soap. On the off week, the caregiver can do the dusting, cleaning of bathroom, living room, bedroom, etc.

Applicants must have the following to apply:
1 year of professional care-giving experience (in-home or facility) or a CNA/HHA license in the state of Tennessee.
2 valid forms of identification (i.e. drivers license, social security)
Show proof of up-to-date auto insurance, have a valid drivers license and access to a reliable vehicle
Must be able to pass a 7 point pre-employment background check
Must be 18 years of age.

CNT's/Caregivers must be able to:
Perform personal care in assisting with activities of daily living which include (but are not limited to):
Personal Hygiene (Bathing/Showering, Hair/Mouth/Skin Care, Grooming)
Ambulation/Transfers
Eating/Feeding
Continence/Incontinence Care and Toileting
Housekeeping/Homemaker Duties (i.e. dusting, vacuuming, sweeping, mopping, meal prep/clean up, clean bathroom, change be linens/laundry).
