The following requirements are needed to contract with us:

• Level 2 background screening on file with AHCA
• Two work/personal references
• Valid driver's license & reliable transportation
• Social Security Card
• Current CPR card
• Proof of TB within the past 12 months
• Communicable Disease Statement dated within the past 6 months
• HIV/Aids CEU
• Infection Control CEU
• Alzheimer's CEU
• Smart phone
