Position Objective : The PAHCG Scheduler/Care Manager will perform a variety of duties in order to coordinate the scheduling of services for clients and caregivers and facilitate communication between clients, caregivers, RN Case Manager and Office staff. The Scheduler/Care Manager is the first line advocate for both the client and the caregiver.

Scheduler/Care Manager Roles & Responsibilites :

Create and maintain client and Caregiver schedules with an emphasis on matching the ideal caregiver to the client's needs. Notify client's family,client,and caregivers of scheduling changes.
On an hourly basis, monitor, log and evaluate caregiver dashboard activity within the EMR system, notify RN Case Manager of any changes in client condition or caregiver concerns.
Contact new clients after services have started to check in on their PAHCG experience, problem solve any scheduling, care plan and/or caregiver changes.
Problem resolution - mitigate, document and determine problems which need to be escalated to management.
Record and maintain accurate and up to date client and caregiver records in EMR system.
Recognize and proactively pursue opportunities to increase caregiver services hours to ensure the best quality care for our clients.
Educate client inquiries for new service over the phone on the PAHCG difference and pricing. Document prospect initial call information into the EMR system, schedule intake/assessment, and put together intake folder with client information for RN Case Manager.
Develops a system to work with the PAHCG HR Recruiter to notify of staffing needs on a weekly basis.
Update HR Recruiter regarding any changes in Caregiver availability, ability to forecast some changes based on feedback from Caregivers.
Develops a system to reduce caregiver overtime and maintain system by consistently evaluating current staffing needs.
Will rotate as on-call supervisor.

Current or past CNA or MA license preferred
1 year Scheduling and Home Health Care experience
1 year of Customer Service experience
On-call experience preferred
Must possess a valid driver s license and pass required background checks
Familiarity with ClearCare preferred
