Position Summary:

Supervise and manage the work performance of 35 to 40 Service Workers (Home Care Aides {HCA s} & Certified Nursing Assistants {C.N.A s}) as it relates to Home Care Services Program. The purpose is to ensure adherence to Performance Quality Improvement (PQI) Standards, Scheduled Services, deadlines, and proper procedures, correcting errors and or problems. Resolve Participant and Home Care Aide/C.N.A complaints and or answer their questions regarding policies and procedures.

The HCS Supervisor must be able to travel throughout the Greater Metropolitan Chicagoland Area including suburbs where Casa Central Home Care Services are provided in order to conduct supervisory home visitations.

Essential Duties:

  • Responsible for creating relationships with participants maintaining and promoting participants self-respect and encourage their independence, when possible.
  • Provides direct assistance to participants of the Home Care Service Department.
  • Make the necessary referrals when appropriate.
  • Assigns a Home Care Aide/C.N.A to a participant within a 15 calendar day and or in 48 hours for an interim referral.
  • Develops an Introductory Visit for new participant in the program.
  • Maintains record of services provided for participants.
  • Provides input to the Care Coordinator on the services that are needed for each participant as a result of conferences with the HCA s/C.N.A s, with courtesy phone calls or in-home visits.
  • Assists participants with process of redeterminations.
  • Reports on home and on-site visits, phone and or office conferences. Files reports in either the participant s case record or the Service personnel file, as appropriate.
  • Provides HCA/C.N.A guidance in handling difficult or complex problems or in resolving escalated complaints or disputes.
  • Implements corporate or departmental policies, procedures, and service standards in conjunction with management.
  • Discuss job performance problems with HCA s/C.N.A s to identify causes and issues and to work on resolving problems.
  • Train HCA/C.N.A on job duties and company policies.
  • Evaluates employees' job performance and conformance to regulations and recommend appropriate personnel action
  • Monitors the provision of services and the HCA s/C.N.A performance through home visits and phone calls.
  • Documents Home Visits on proper forms and participant file.
  • Conducts quarterly Home Visits with each participant in service area assigned, averaging 7 Home Visits per visiting day.
  • Prepares a schedule assigning each HCA/C.N.A to a participant.
  • On the fifth of each month, completes monthly reports which consist of, Home Visit log, new participants, new HCA s/C.N.A s assigned, high risk participants, hours provided, performance evaluations and quarterlies.
  • Manages each HCA s/C.N.A s work hours using HomeTrak scheduling system.
  • Documents contact and quarterly conferences with each assigned HCA/C.N.A.
  • Prepares and reviews participant s Plan of Care as recommended by Case Coordinator Unit (CCU) in order to instruct HCA/C.N.A on essential duties to be rendered for the participants.
  • Collects Calendars from HCA s/C.N.A s including confirming accuracy of service hours worked per respective participants and turns in to Billing Specialist.
  • Documents reports in either the participant s case record and or the HCA/CNA personnel file, as appropriate.
  • Reviews records and reports pertaining to activities such as services, monitors work activities, quarterly s conferences, home visits, and performance evaluation.
  • Interviews and makes recommendations of new HCA s/C.N.A s.
  • Monitors and provides direct supervision to 35 to 40 Home Care Aides (HCA s) and Certified Nursing assistants:
  • Interpret and communicate work procedures and company policies to HCA s/C.N.A s.
  • Communicates administrative policies and procedures.
  • Implements the human resources policies, procedures and practices of the organization
  • Must attend training and staff meetings when required.
  • Performs other duties and responsibilities, as assigned by the Program Director or Deputy Director of Senior and Community Services.






  • Sensitivity to cultural diversity is required. Will be subject to drug and alcohol screening, as well as aptitude and skills testing. To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to perform the essential functions. Must have a basic knowledge of home management skills. Will be required to travel throughout Chicago to conduct introductory meetings and on occasions on-site supervision of the staff at client s homes; will need personal vehicle and valid Driver s License and vehicle insurance.
  • Bachelor s Degree from an accredited college or university and/or three (3) years of experience in case management or senior service related field; with a combination of skills and experience which indicates that the applicant has the ability to perform the supervisory activities. At least one year of experience working with the elderly population is preferred.
  • Bilingual / Bicultural (English & Spanish); Must possess excellent communication skills, oral and written. Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Microsoft Word, Excel, Outlook and the ability to learn Time & Labor Software.
  • HCS Supervisor will travel to conduct required home visits as necessary; additionally, will interact with participants and Home Care Aides on a regular basis. Will contact the other Departments of the agency, case coordination unit, Department on Aging, private organizations, social service agencies and community groups if necessary.

  • HCS Supervisor should report to the Senior Supervisor any concerns that may arise. In situations involving major and or imminent risk to either a Home Care Aide and or a participant the HCS Supervisor must consult immediately with the Program Director.



  • Managerial/supervisory skills required and must be able to follow through on all tasks. Must have ability to deal with older adults, being sensitive and responsive to cultural differences. Must have a good understanding of the elderly population and their needs. Must assists in the Supervision of the Service Workers of the program and of the service provided to participants.
