The Traveling Clinical Supervisor is a registered nurse who manages the coordination, supervision and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Traveling Clinical Supervisor is responsible for the coordination of care during the episode of health care treatments and will provide education and training related to clinical practice issues, regulation and reimbursement changes with new employees during the preceptor phase of new hire orientation. The Traveling Clinical Supervisor will receive temporary assignment from the Executive Director based upon branch staffing needs. This position requires the ability to build rapport quickly, and the flexibility to be comfortable with adjusting to a different work and living environment on a consistent basis.
My commitment will include:
  • Communicating significant findings, problems and changes in condition or environment to the physician and other personnel involved with patient care.
  • Initiating a plan for patient safety using the patient, family and community resources.
  • Verifying the plan of care for the episode according to physiciana??s orders, assessment data and established standards and guidelines.
  • Managing and directing a team of field staff to ensure the delivery of safe, effective and appropriate home care services.
  • Facilitating admission reviews and case managing for a team of patients.
  • Supervising the coordination of services with providers in the delivery of care and related issues.
  • Processing and coordinating patient referrals, physician orders, staff assignments, patient re-certifications and communications between various caregivers, the patient and their family and the referring physician.
  • Instructing and evaluating the field staff with special patient care procedures in accordance with the scope of practice and policies of the agency.
  • Identifying and instructing staff on admission criteria for new clients and evaluating eligibility for home health care benefits.
  • Assuring compliance with the requirements of state licensure, Medicare certification and any other applicable oversight agencies through policy and procedure development, staff education and ongoing monitoring activities.
Qualifications:
  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. BSN preferred.
  • Licensure: Current State License as a Registered Nurse. Licensing required in all states served by Great Lakes Caring.
  • Experience: 2-3 yearsa?? experience as a registered nurse in an appropriate clinical care setting; home health environment preferred. Evidence of clinical and supervisory knowledge and experience appropriate to the assigned supervision responsibilities.
  • Skills: Nursing skills as defined by accepted nursing standards. Requires interpersonal skills and ability to manage patient care episodes.
