Registered Nurse - Part-time

Job Description: Clinical RN As part of the Confluence direct-care team, the Clinical Nurse plays an important role overseeing and maintaining the physical health of participants. The Clinical Nurse is an integral link in the spectrum of services offered at Confluence. This person works directly with participants to help maintain wellness and address health concerns as they arise. The Clinical Nurse also works as a member of the treatment team working closely with direct-care staff, therapists and clinical director and the health services director.   Responsibilities:       Conducts initial Nursing Assessments on all new participants and initiates a plan of care      Conducts bi-weekly health reviews for participants, maintaining data on biometrics and documents any reported symptoms or health concerns      Manage daily medication assistance processes assuring that all participant medications are correctly packaged, available, and appropriately documented and accounted according to regulations and policies and procedures      Liaises with outside physicians/pharmacies to manage prescriptions and the physical health of program participants      Trains and assists staff who provide medication assistance      Liases with Clinical Team to address concerns regarding participants physical health      Provides monitoring and education to participants on best practices to improve nutrition, sleep, activity and weight management      Makes appropriate referrals to outside health professionals for any health conditionsValley News- Lebanon NH.
