The Summit Community Care Clinic (SCCC) Staff Physician is responsible for the delivery of professional medical services, supervision of staff and volunteer medical providers and participating in Quality Assurance program. The Physician must demonstrate regard for the rights of all patients and provides respectful and considerate care focused on the patient's individual needs. The Physician will provide a safe and confidential environment for patients, families and personnel. Essential Functions: Promotes the philosophy and mission of Summit Community Care Clinic. Provides direct patient care following established protocols, including but not limited to: conducting outpatient clinic; establishing patient history through interview; establishing diagnostic impression; managing chronic health problems; initiating arrangements for treatment. Makes decisions concerning medical care and the needs of the patient, as well as decisions regarding special needs, following established protocols. The delivery of professional primary care services on behalf of SCCC, within the scope of training and expertise of the Employee. Hours and number of patients scheduled will be mutually determined by Employee and SCCC. Minimum of 16 hours is required for current Part Time position, 30 hours minimum is required for Full Time position. Participation in the Quality Assurance Program of SCCC, including attendance at QA Committee meetings and Peer Review processes, as appropriate. Actively participates as a member of the Integrated Care team. Works directly and through referrals with therapist and psychiatrist to evaluate/treat mental health disorders of clients. Refers and collaborates with community mental health agencies and counselors as appropriate. Ensures appropriate documentation and maintenance of medical records. Assists with clinical supervision of mid-level providers. Performs other related job duties as assigned by Executive Director. Reviews patient file/record, including allergies, problems, medications, and immunization status. Elicits and records information about patient's medical history. Examines patients for symptoms or physical information. Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition. Analyzes reports and findings of tests and examination, and diagnoses condition of patient. Administers or prescribes treatments. Determines and prescribes medication, dosage, and schedule given the patient's condition and allergies. Discusses any possible side affects to medication or immunization with patient. Prescribes and administers vaccinations to immunize patient from communicable diseases. Promotes health by advising patients about diet, hygiene, and methods for prevention of disease. Provides prenatal care to pregnant women, delivers babies, and provides postnatal care to mothers and infants. Performs surgical procedures commensurate with surgical competency. Refers patients to medical specialist for consultant services when necessary for patient's well-being. Documents the patient's visit including medical history, physical exam, diagnoses, and plan of action. Follows up with patient regarding progress in high risk or emergency cases. Conducts physical examinations to provide information needed for admission to school, consideration for jobs, or eligibility for insurance coverage. YourMembership.
