Aerotek is currently hiring for full-time LCSW-C and LCPC openings in the Maryland area. This is an immediate position with one of our best clients starting on November 6, 2017.
LCSW-C/LCPC Job Description:
This is a telephonic LCSW-C or LCPC position designed to work with the health plan to increase the integration of physical and behavioral health care. The LCSW-C or LCPC will provide a one stop access point for member's behavioral health needs. They will be the point of contact for the health plan and network physicians for behavioral health needs and will direct behavioral health requests to the correct team/individual and report back to the health plan and/or physician practice on the status of those requests. The LCSW-C or LCPC will also interact with facilities and other community resources to address individual member need and improve the service delivery system. The LCSW-C or LCPC will be responsible for explaining the health plan services, complete intake assessments for patients and their health assessments, providing treatment plans, and moving patients from illness into recovery phase. Patients will be under care anywhere from 3 months-8 months, building relationships with the patient via phone and keeping them on track with the treatment plans.
Hours: Monday-Thursday 8 am-7 pm and Friday 8 am -6 pm. 2 evening shifts/week are required (evening shift defined as staying until 7 pm Mon-Thurs or staying until 6 pm on Friday). There is flexibility with the schedule!
Qualifications: Active LCSW-C or LCPC with a minimum of 5 years of post-master's experience.
