Position Summary:

This position reports to the Market Manager of Pharmacy Services and is an advanced clinical pharmacist role who responsible for providing clinical pharmacy services to meet the needs of patients and support the mission of the Department of Pharmacy, Hospital, and Health System. Provides comprehensive pharmacy care through protocol/guideline management as well as individualizes drug therapies to promote safe, efficacious and cost-effective patient care. Serves as a role model for Clinical Pharmacists I, pharmacy residents and fellows, as well as pharmacy students. He/she creates and contributes to the ongoing education and development of pharmacy personnel and the education of other health care practitioners.

Position Responsibilities:

* Follow departmental procedures for checking the accuracy of the pharmacy technical personnel in assigned areas.

* May act as the liaison between pharmacy services and other departments and/or participate on committee meetings as a department representative.

* Identify performance improvement opportunities and make recommendations of ways to improve clinical and distributive services.

* Proactively work to improve the medication process.

* Ensure the accuracy of records pertaining to controlled substances, medication compounding, prepackaging, and sterile product preparation.

* Collaborates with health care providers to individualize medication therapies to promote safe, efficacious, and cost-effective patient care following best practices and evidence-based guidelines.

* Develops, Implements, and evaluates quality improvement and clinical pathways/programs related to medication use to ensure optimal patient care.

* Contributes to formulary management and works with health care providers to ensure formulary compliance.

* Actively Participates in medical staff and service line meetings as requested, including Pharmacy & Therapeutics Committee, to meet patient care needs and Service Line objectives.

* Assumes a leadership role in Service Line initiatives as related to medications and the medication use process.

* Develops, Implements and Leads projected cost savings/cost avoidance related to medication utilization and Provides routine reports on status.

* Participates in Strategic and Operational Planning activities and the development of the Pharmacy Department's annual budget.

* Educates, mentors, and evaluates pharmacists, pharmacy residents, and/or pharmacy students to develop and improve skills necessary for providing pharmaceutical care.

* Develops and Provides educational programs, learning modules, and knowledge assessment tools.

* Educates other health care professionals regarding medications to promote optimal patient care via in-services, educational programs, face-to-face interactions, etc.

* Reviews, interprets, and evaluates all medication orders for patients in all age groups.

* Maintains patient medication profiles in an automated pharmacy information system.

* Consults with and advises medical, nursing, and other healthcare providers regarding the prescribing, dispensing, or administration of medications.

* Routinely monitors the patient's total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.

* Serves as a pharmacy resource in cardiopulmonary emergencies.

* Conducts assigned target drug

* Performs and documents Pharmacist Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy

* Obtains medication histories on selected patients and communicates pertinent information to the appropriate healthcare provider.

* Communicates with physicians, nurses, and other healthcare providers as necessary.

* Prepares and/or Verifies the appropriate preparation of medications to be dispensed using the correct technique, as required.

* Provides staffing coverage

* Provides on-call pharmacy clinical support as required.

Minimum Qualifications:

Education Level

BS Pharm or Pharm D

Licensure/Certification

Texas pharmacist license

Complete an ACPE approved 20 hour sterile products course

Knowledge, Skills, and Abilities

Experience with patient education

Relevant knowledge and skills with managing pharmaceutical kinetics and drug therapy monitoring

Experience

Minimum

Two (2) Years of experience as a clinical pharmacist in an acute care setting; or

Preferred

Master's degree in business administration or similar field of study

Completion of pharmacy administration residency

Completion of pharmacy (PGY-1)residency
