For each section (1a, 1b, etc.) you must do the following:

Please note: Mission, Vision, Values evaluates behaviors all associates are expected to model. Lourdes Health System has a wide variety of positions, both clinical and non-clinical; therefore, not all competencies will apply to all positions in the same manner.

The focus of Mission/Vision/Values is "how did the associate perform with respect to our organization's core values?"

The statements shaded in gray are the major criteria for determining performance.

The items found under Performance Needs Improvement, Meets Expectations and Demonstrates Excellence are suggestions, examples and discussion topics regarding the types of behaviors normally considered when evaluating an associate. They are intended to prompt conversation between an associate and their manager. Not all of these items will apply to all positions in the same manner.

Under the Job Specific Responsibilities and Competencies Section, the items under Performance Needs Improvement, Meets Expectations and Demonstrates Excellence are reviewed in more detail, since they related directly to the specific position being reviewed.

You should not score an associate based on tallying each individual bullet. Scoring should be based on the associate's overall performance in the particular category.

This tool will also be utilized to identify training and development opportunities.

Scoring Definitions

1 = Performance Needs Significant Improvement: associate performance meets only a few of the performance expectations. Associate needs significant improvement to maintain employment. PIP required

2 = Performance Is Below Expectations: associate performance meets many expectations but is deficient in some areas. Needs continued improvement. Performance issues have been identified and communicated. Issues not resolved by the next performance appraisal require PIP.

3 = Performance Meets Expectations: associate performance meets performance expectations. Good solid performer who consistently meets the high standards established by the department. The Health System expects work results that are above average and an associate in this category has met those expectations. A majority of associates will fall into this category.

4 = Performance Is Above Expectations: associate performance exceeds expectations in most areas most of the time. A level above the high performance expected from all associates, associates in this category consistently exceed expectations.

5 = Performance Demonstrates Excellence: associate performance far exceeds expectations. Performance is significantly above peers. Very few associates reach this level. Serves as a role model for the organization.
