Providence is calling a Part Time, Night Shift Care Manager to Providence Little Company of Mary Medical Center in Torrance, CA.
In this position you will:
* Manage patient care, to achieve cost-effective utilization of resources through the proactive coordination of services in order to move patients efficiently through the continuum of care
* Assume responsibility for an interdisciplinary process which assesses, plans, implements, monitors the effectiveness of interventions to meet patients' treatment and transition of care needs
Required qualifications for this position include:
* Nursing degree/diploma required upon hire.
* All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree in Nursing or higher within 3 years of hire.
* Any previously agreed upon effective date for obtaining a Bachelor's degree in nursing between individual and facility will remain in effect.
* Any collective bargaining contract with specific time frames for obtaining a Bachelor's degree in nursing will supersede the above point and will remain in effect.
* Exceptions will be considered on a case-by-case basis at each facility by the Director of Care Management in consultation with the Human Resources leader.
* A RN with 20 years of RN experience will not be required to obtain a BSN
* RNs employed prior to 4/1/17 are encouraged to obtain a Bachelor's degree or higher degree in Nursing, but obtaining the degre will not be a condition of employment.
* Current California Registered Nurse license
* Current Basic Life Support Card (BLS) for Healthcare Providers sponsored by the American Heart Association
* Minimum of one year experience within the past three years in an acute hospital setting as Registered Nurse.
* Minimum of one year experienc as a Discharge Planner or Case Manager, succesful completion of a minimum of 24 CEUs in Care Management within the past 12 months, or successful completion of the Transition into Practice s(TIP) program for Care Management
* Strong clinical knowledge base
* Advanced knowledge of McKesson's InterQual SI/IS Criteria
Preferred qualifications for this position include:
* Bachelor's Degree in Nursing (BSN) strongly preferred
* Masters Degree
* Minimum of 3 years experience in case management, discharge planning, and/or utilization review
