POSITION SUMMARY

The Blood and Marrow Transplant Coordinator coordinates all services and systems to provide an organized, multi-disciplinary team approach for the care of patients within the blood and marrow transplant service. Participates in the development and implementation of clinical programs and procedures and participates in the performance improvement program. Coordinates the entry and integration of prospective BMT patients into and through the BMT program while serving as a clinical and educational consultant to the nursing staff, medical staff, management, and other members of the health care team regarding the management of BMT patients.

MINIMUM POSITION REQUIREMENTS

Education/including Licensure/ Certification/ Registration

RN Licensure - State of California is required

BLS -Health Care Provider is required

Graduate of an accredited school of nursing

Nursing\Certified Oncology RN - CN-ONCLGY is required within 1 1/2 years

Blood and Marrow Transplant Certification (BMTCN) is required within 1 1/2 years

Skills/Knowledge:

Skills in patient education, leadership, consulting, teamwork and collaboration are necessary.

Interpersonal and communication skills, both oral and written are required.

Ability to conduct simple statistical analysis is required.

Department Specific Skills and Knowledge:

Specialized clinical knowledge demonstrated in the field of blood and marrow transplantation, including autologous and allogeneic transplant process, Graft-vs-Host Disease, immunology, educational strategies and the knowledge of the unique needs of the specialty population served.

Demonstrated knowledge of the coordination of services of a multi-disciplinary team is necessary.

Demonstrated knowledge of patient education, program planning, reimbursement and contracting issues related to third party payers is desirable.

3-5 years of significant clinical experience in the field of blood and marrow transplantation is required including knowledge in both autologous and allogeneic transplants, to include cord blood transplantation is required. Case management experience is desired. Program development, administration, coordination, education, management and research experience are desirable.

On-call/standby is required every other weekend.

COORDINATES THE ENTRY AND INTEGRATION OF PATIENTS INTO AND THROUGH THE BMT PROGRAM.

* Participates in the evaluation of patient candidates for BMT by providing a weekly candidate list and presenting information about candidates at weekly multi-disciplinary steering committee meeting

* Performs an assessment of the patient's and family's readiness for transplantation, utilizing specialized clinical knowledge to assess the physiologic and psychosocial needs of the patient.

* Educates patients, their families, and donors to assist understanding of the disease, processes and procedures surrounding pre- and post-transplant concerns.

* Acts as a resource for members of the interdisciplinary team on transplant care and protocols to promote positive outcomes for BMT patients.

* Assures patient preparation for admission by coordinating, scheduling, and obtaining results from the appropriate lab tests, procedures, consultations, and financial screens including appropriate lab studies done on donors and recipients for allogeneic transplants.

* Assures patient preparation for collection of stem cells by coordinating, scheduling, and obtaining results from the appropriate lab tests, procedures, consultations, and financial screens including appropriate lab studies done on donors and recipients for allogeneic transplants.

* Assures appropriate consents are obtained.

* During allogeneic transplants, coordinates donor and recipient identification and acts as resource to the recipient, donor, medical staff, and transplant staff regarding donation and allogeneic transplantation with education and scheduling.

* Coordinates care of National Marrow Donor patients for their harvest/collection procedure

COORDINATES PROGRAM DEVELOPMENT AND INTEGRATION OF SERVICES AND FINANCIAL

OPERATION.

* Participates in the development of clinical pathways through clinical expertise and communication with multidisciplinary team.

* Acts as liaison between Sutter and Blood Source for the National Marrow Donor Program.

* Participates in strategies to integrate program elements within Sutter Medical Center and other Sutter affiliates.

* Participates in BMT program development in areas directly related to patient care.

* Identifies ways to decrease costs while maintaining quality of care.

* Participates in ensuring that clinical programs meet or exceed requirements of accrediting, licensing, and regulatory agencies.

* Assists in getting third party payer contracts by assisting in RFP's.

* Assists in the development of BMT promotional materials and promotional campaigns.

COORDINATES AND MAINTAINS AN ONGOING PROCESS IMPROVEMENT PROGRAM.

* Works directly with the Clinic Program Quality Manager and Bone Marrow Transplant (BMT) staff to collect statistical data.

* Incorporates data and information to continuously improve care and practice.

* Uses the data for quality assurance activities including, morbidity and mortality information, engraftment, continuous resource management, and internal quality initiatives.

* The individual in this position prepares weekly, monthly and yearly statistical analysis and reports and submits them to the Clinical Program Quality Manager.

* Assures the integration of a multi-disciplinary team approach throughout all phases of the BMT process, which includes screening, evaluation, patient selection and preparation, intra-transplant, supportive care and post-discharge.

* Works cooperatively with the Clinical Program Quality Manager to evaluate BMT program for efficiency, safety and effectiveness of standards and protocols.

* Compiles quarterly reports of measures and activities for the program elements, reports results of monitoring and evaluation activities to the BMT Clinical Program Quality Manager

* Participates in interdepartmental and Performance Improvement committees as assigned

* Coordinates and analyses data and clinical status information of patients referred to Bone Marrow Transplant Program.

* Participates in the quality improvement process.

* Provide Education to staff and Physicians related to improvement opportunities in the documentation for both BMT database and CIBMTR forms.

* Reviews medical records for data collection and analyses to meet requirements for necessary forms, completes data entry.

* Maintains applicable spreadsheets of all CIBMTR TED forms, research forms and NMDP protocol and consents.

* Assist with long term follow up process.

* Assist in the coordination and preparation for on-site surveys and/or RFI (Request for Information) as received by various accreditation agencies under the direct supervision of the BMT Clinical Program Quality Manager.

* Monitors and ensures compliance in the utilization of research consents.

DEMONSTRATES CLINICAL EXPERTISE AND PROVIDES CONSULTATION/COMMUNICATION

* Participates in patient care by serving as role model, and assisting with identification of the educational needs of staff, enhancing interdisciplinary relationships, and researching and resolving patient care issues.

* Maintains knowledge of clinical nursing standards and uses and implements standards in assisting in the development of protocols and procedures.

* Integrates and coordinates follow-up ambulatory care of BMT patients. Identifies a variety of options for hospital/community resources to meet patient/family needs at the time of discharge and institutes appropriate referrals.

* Communicates directly with post-discharge care givers for problem identification and reentry into the program when necessary.

* Utilizes a wide range of approaches to explain and anticipate patient and family problems in order to establish and help others establish priorities of care.

* Collaborates with appropriate people from the multidisciplinary team in form development and drafting of guidelines for documentation of care.

* Abides by structure standards of BMT nursing unit.

* Keeps BMT Medical Director and BMT Clinical Program Quality Manager informed of plans and activities.

* Maintains a current clinical knowledge base by attending BMT-related seminars, Webinars, and completing BMT-related classes.

* Cooperates/collaborates with representatives from other health care facilities as appropriate with education material and scheduling to ensure patient is being cared for consistently across the continuum of care.

EDUCATION

* Ensures improvements in practice by assuming responsibility for self-development and lifelong learning.

* Work closely with patients, families, physicians, and members of the outpatient and inpatient nursing staffs, and other health care team members in formulating instructional patient care and teaching plans

* Uses knowledge of specific clinical needs of bone marrow patients when developing educational materials.

* Provides transplant information regarding pre-transplant, the transplant procedure, and post-transplant issues to patients, families, donors, and caregivers.

* In conjunction with nursing staff, medical staff, and management, establishes a staff BMT education program for orientation and continuing education.

* Assists and participates in the planning and implementation of conferences, seminars, and other educational offerings within and outside of Sutter Health, related to BMT.

* Uses established educational methods.

* Develops educational resource materials.
