For 17 years Home Instead Senior Care of Charlotte has enriched the lives of seniors in Mecklenburg County. We are currently seeking a Client Care Coordinator who has a passion for seniors to join our team. We are looking for an organized individual to join us in our mission to enhance the lives of aging adults and their families. This position involves managing quality assurance with extensive one-on-one interaction with clients and families. This person is responsible for effectively managing multiple demands that come with the changing needs of the clients. This individual must be able to work in a fast-paced organization and possess excellent communication skills, be service-oriented and have the ability to work well in a team environment. 

Responsibilities
  • Meet potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Maintain ongoing communication and a consistent follow-up schedule with clients and their families.
  • Work with other team members to coordinate various aspects of a client's care
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of all clients through a quality assurance program.
​​​Qualifications
  • College degree preferred
  • Licensed Registered Nurse (RN) preferred
  • 3+ years experience in  home care, health care or senior-related industry required and equivalent combination of education and work experience may be considered.
  • Excellent oral and written communication skills and the ability to listen effectively
  • Ability to work independently, maintain confidentiality of information and meet deadlines. 
  • Demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Ability to work evenings and weekends when necessary
