Job Title: CNA

Job Reporting Relationships:

Job Purpose:

Under the direction and supervision of the Day Center Manager, this individual performs ADLs and activities within the scope of practice of a Certified Nursing Assistant, such as clinical observation, clinical reporting requirements, participant laundry, participant personal care, participant care training and education. It is the responsibility of the Day Center CNA to provide and assist the participants with activities of daily living, restorative and supportive care. Monitors health status and provides a safe environment for participants. Assists with organization and implementation of activity program. Functions as a member of the interdisciplinary team on an as needed basis. Accepts other duties as assigned by the Day Center Manager and within the CNA cope of practice.

Skills:

* Knowledge of principles, practices, standards and techniques of a CNA

* Knowledge of common safety hazards and precautions for a safe work environment

* Knowledge of medical equipment use and maintenance

* Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff and the public

Major Duties and Responsibilities:

1)Teamwork/Communication.

a. Works as a team member. Contributes to team discussions and willingly accepts direction

from team members. Responds flexibly to program and participants' need.

b. Collaborates with the team to plan, implement and evaluate participants' plans of care.

2)Contributes to participant and caregiver satisfaction by providing high quality services and communicating effectively when participants or families have concerns or questions regarding home care services. Understands customer service. Refers concerns to supervisor.

3)Provides assistance with activities of daily living (ADL and IADLs) for participants in their homes or in the center in accordance with the personal care plan developed by the team. This may include but is not limited to:

Assistance with mobility, including contact guard during ambulation, transfers, wheelchair

mobility, lift transfers.

Assistance with bathing and grooming, including shower or tub baths, bed baths, peri-care,

shaving, hair washing, dental care, nail care

Assistance with toileting.

Assistance with dressing.

Assistance with cuing to eat, or feeding.

Meal preparation and clean-up

Cuing to take medications

Light housekeeping, laundry

Occasional light shopping

4)Reports and when appropriate, records any changes observed in the condition or behavior of

the participant and any unusual incidents to the Day Center Manager or designee.

5)Provides general assistance and support when requested by supervisor or designee.

6)Prepares and serves meals and cleans up kitchen and dining area.

7)Escorts participants on the bus and to and from appointments.

8)Engages the participant in recreational programs as directed by the Recreational Therapist.

9)All other duties as assigned.

10)Acts within the scope of his or her authority to practice.

Minimum Qualifications:

Education:

High School diploma or GED.

Certified Nurse's Aide (CNA) with current certification, Certified or Registered Medical Assistant, or a trained home health aide meeting the competencies in Delaware (must provide verified documentation of training and competencies)

Experience: CPR trained or willingness to be trained. Long Term Care and/or Home Care experience preferred, but not required.

Minimum of one year experience in working with the frail or elderly population

Possess:

Ability to communicate well in English, orally and in writing.

Ability to interpret and follow oral and written instructions.

Understanding of importance of dignity and privacy in personal care for older adults.

Medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Valid driver's license and daily availability of a car.
