Position Purpose:
Job Description Details:
I Accountability Objectives:
Provides primary health care to patients through the identification, management and/or referral of health problems as
well as through health maintenance interventions related to illness, prevention and health promotion. Collaborates
with the Medical Director to provide programmatic supervision and day to day operational management. Identifies
staff development needs and provides leadership to other members of the health care team.
II Position Qualifications:
Minimum Education, Licensure / Certification and Experience Required.
A. Education
Masters of Science in Nursing required.
Graduate of accredited Advanced Practice Nurse program.
B. Licensure / Certification
Current Basic Life Support certification (BLS)
ANA Certified in Family Nurse or Adult Nurse practice.
Current licensure as a Nurse Practitioner in the State of Michigan.
C Special Skill / Aptitudes
Excellent customer service orientation skills necessary in order to deal effectively with various levels of
hospital personnel, outside customers and community groups.
D. Experience
Three to five years clinical working experience required.
Bilingual in Spanish/English preferred.
III Duties / Responsibilities:
1 Coordinates provision of patient care in collaboration with all members of the interdisciplinary team. This
includes assessment of patient/family needs, development of plan of care, and reassessment of effectiveness of
care with modification of the plan of care as needed.
2 Assesses real or potential health hazards and current health status, including analysis of health behavior
related to lifestyle and culture.
3 Develops, implements and evaluates plans of care utilizing sound clinical judgements based on assessment of
the physical, psychological, emotional, societal and environmental needs of the clients.
4 Assists individuals to assume responsibility for prevention of illness and promotion, maintenance and
restoration of health.
5 Demonstrates competency in the performance of specific procedures delegated by the physician. These
procedures include but are not necessarily limited to:
a) venipuncture
b) PAP/Pelvic
c) History and Physical
d) Rectal exam
e) Suture removal
6 Utilizes the resources of St. Joseph Mercy-Oakland and the surrounding area community as appropriate.
7 Assists in the establishment and accomplishment of the departmental and unit goals and objectives through
specific assignments and projects.
8 Maintains knowledge of current trends and developments as well as legislative issues to nursing practice and
health care by reading appropriate books, journals and other literature and attending related seminars,
conferences, etc.
9 May participate in community education and other professional activities.
10 May assist in the preparation of the annual budget. Reviews budget reports on a regular basis to identify and
address variances, and identify opportunities for fund raising.
Demonstrates and actively promotes an understanding and commitment to the mission of St. Joseph Mercy
Oakland through performing behaviors consistent with the Trinity Health Values.
Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity
Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as

Supports and conducts one's self in a manner consistent with customer service expectations.
In accordance with unit or departmental practice, determines that appropriate charges have been entered for the
correct patient, encounter, date/time of service, with any required modifiers. Make corrections as needed per
charge capture policy/practice.
In accordance with unit or departmental practice, actively participates in on-going education and
communication regarding revenue management.
In accordance with unit or departmental practice, assists with tracking and monitoring of equipment assigned
to the unit. Requests services for maintaining equipment as needed.
In accordance with unit or departmental practice, proficient with the electronic health records for
documentation, assessment, and care management, performing these activities concurrently with provision of
care throughout the shift.
V Dept/Unit Specific:
* May participates in the competency assessment of associates, providing feedback when appropriate and
collaborating with the Director liaison to complete annual evaluation.
* May serve as a liaison to the community and is involved in identifying the need for additional
programs/services.
* May collaborates with and serves as a liaison to community agencies including but not limited to: Oakland
County Health Department, Oakland Family Services, Lighthouse, United Way, Haven, etc.
V Working Conditions:
* Working with patients may result in exposure to communicable diseases and hazardous substances.
