Position Purpose:

The Registered Nurse supervises the delivery of nursing care provided by professional and support nursing staff on a designated unit. Care is provided in a manner consistent with the philosophy and objectives of Providence Hospital and the ANA Standards of Practice for Psychiatric Mental Health Care. The Registered Nurse prescribes, coordinates and delegates care utilizing the nursing process which is integrated into the multidisciplinary treatment team plan of care. The Registered Nurse is accountable for nursing care activities on a shift basis and is responsible for promoting and enhancing professional nursing practice on the unit.

Job Description Details:

PRIMARY JOB RESPONSIBILITIES AND DUTIES:

* Performs comprehensive nursing assessment on all patients inclusive of the bio-psychosocial and medical history with consideration to age, mental, social and physical status and response.

* Identifies patient problems based upon data collected through nursing assessment as well as integrating information obtained through family, significant other and/or primary care giver.

* Formulates nursing plan of care noting patient's needs, age, behaviors and concerns identified through nursing assessment.

* Analyzes data and develops realistic and measurable short and long term goals and desired outcomes incorporating patient's and family's goals, when appropriate.

* Establishes priorities for nursing needs based on nursing assessment, problems identified and estimated length of stay.

* Ensures nursing plan of care is consistent with the master treatment plan, making appropriate revisions, as needed.

* Acts as a liaison between the treatment team and nursing staff, ensuring accurate communication of relevant information.

* Demonstrates active collaboration with Treatment Team in anticipating discharge needs of assigned patients, i.e. follow-up medical appointments, need for VNA linkage.

* Practices within the general policies, philosophy and objectives of the Department of Nursing and the hospital.

* Participates in community meetings and psycho-educational groups.

* Assesses unit milieu and maintains a safe, therapeutic environment for patients, family members and staff. Modifies milieu according to developmental needs of the patients.

* Assures activities of daily living needs of patients and considers developmental needs of children, adolescents, adults and geriatrics in process of ADL.

* Assesses patients in crisis and acts according to developmental needs of the patient to ensure patient and unit safety.

* Provides ongoing patient/family education.

* Transcribes and administers medication and treatment orders and co-signs all orders in accordance with Medication Policy.

* Evaluates effectiveness of nursing care.

* Performs independent problem solving and decision making in a logical and deliberate manner, utilizing available resources when needed.

* Assumes responsibility for personal and professional growth and development through participation at in-services, workshops, staff meetings and literature reviews.

* Performs Charge Nurse responsibilities when needed. Assesses the needs to patient prior to delegating tasks to UAP's (Unlicensed Assistive Personnel).

* Actively functions as preceptor for clinical nurses.

* Serves as a role model for staff through maintaining a professional approach and providing a positive work environment and team spirit.

* Actively integrates developmental age and growth theory into all aspects of patient care.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

* To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including children, adolescents, adults, older adults and dually diagnosed patients.

* Graduate of an accredited nursing program. BSN preferred.

* Licensure and registration by the Massachusetts Board of Nursing as a registered nurse. Credentialed in area of practice preferred.

* One year psychiatric or substance abuse nursing experience preferred. Six months to one year medical/surgical nursing strongly recommended.

* CPI certified, or similar applied non-violence skills training, within 30 days of hire. (Does not include the Methadone Maintenance Program).

* CPR certified within 30 days of hire.

* Must possess knowledge of general and psychiatric nursing processes, therapeutic relationships and processes, age-specific growth and development, limit setting, behavior and crisis management and CPR training.

WORKING CONDITIONS:

* Requires considerable physical effort on a consistent basis as in lifting, pulling or pushing objects (heavy, bulky, awkward, more than 40 lbs.).

* Constant exposure to unpleasant tasks (handling garbage or soiled linen, strongly unpleasant odors, etc.) and occasional exposure to one or more very unpleasant physical working conditions; OR occasional exposure to serious injury (crushed fingers, extensive burns, broken bones); and regular and routine exposure to occupational hazards or contagious disease which requires special precautions.

* FREQUENT: Home and social life inconvenience, even though predictable on most occasions, or regular volume of work and deadlines impose strain on routine basis, or work location imposes frequent unwelcome interruptions.

* Failure to follow established procedures could result in injury to self or others, such as: cut, punctures, exposure to bodily fluids, falls on slippery floors, or back injuries from lifting.

* Exposure to patients with varying degrees of psychopathology and who may be potentially aggressive and assaultive; risk of injury from these patients. May have high levels of stress on a frequent basis.

REPORTING RELATIONSHIPS:

* Reports to Nurse Manager.

* Has no responsibility for the direct supervision of any employees.
