Job Description Details:

This Registered Nurse (RN) provides clinical nursing support to the Community Outreach Mobile Coach & Street Medicine Program. This colleague serves as an RN in a variety of community clinical settings; providing assessment, planning, and implementation of care for the patient. They provide education and referrals to the patient as appropriate, and coordinates care with both the physician/nurse practitioner and case worker to assure continuity of care.

Responsibilities

* Appropriately and accurately documents patient information using both chart and computer programs as appropriate

* Functions as a healthcare teacher for the patient and family as appropriate

* Maintains confidentiality with regard to patient information

* Functions as an independent practitioner: planning, directing, implementing, and evaluating care

* Effectively utilizes knowledge of community resources and services to meet patient needs and consistently increases knowledge base of available resources

* Maintains CPR and AED certifications

* Functions as the Charge nurse of assigned clinics, including but not limited to: preparing for clinic, maintaining clinic flow, assisting the physician/nurse practitioner with care on the mobile unit, and completing documentation, as well as follow up of clinic patients

* Coordinates with Mobile Coach team including nurses, physicians, nurse practitioners, case workers, and support staff to provide optimal care of the patient

* Functions as a liaison for the patient with other community agencies and healthcare agencies

* Practices nursing within the scope of the Ohio Nurse Practice Act and functions within approved Mount Carmel Health System and department policies and procedures

* All other duties as assigned

Requirements

* Education: Graduate of an accredited school of nursing

* Licensure / Certification:

* In good standing with a current license to practice as a Registered Nurse (RN) in the State of Ohio

* Currently holds CPR certification or receive training within 6 months of hire date.

* Three (3) years current nursing experience; preferably with experience in medical/surgical, critical care assessment, or community health nursing

* Must be competent in computer use, and use Microsoft programs as well as learn the Mount Carmel electronic medical records systems and other programs

* Willingness to work with all cultural and socio-economic groups

* Demonstrated ability to assess problematic situations and initiate appropriate plan of action in an environment which has limited support systems

* Effective visual, auditory and tactile perception skills

* Knowledge of or ability to learn local community resource/service agencies which provide assistance to identified clientele

* Must possess a current Ohio Driver's License; Ability to independently drive a vehicle to clinic as needed

* Availability to work flexible hours when required
