St Joseph Mercy Home Care & Hospice is a certified Home Health Care and Hospice agency afiliated with Saint Joseph Mercy Hospital. Basic Job Function Summary: Position is responsible for triaging patient/family phone calls, obtaining and processing referrals, and communicating patient needs to on-call visit staff or primary case manager for home care and hospice.

Minimal Qualifications:

* Graduate of an approved Nursing education program

* Current Registered Nurse licensure in the state of practice.

* Must have a minimum of one (1) year experience as a professional acute care nurse.

* Home healthcare/hospice experience preferred.

* Must have current Driver's license and reliable transportation to and from work site.

* Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
