Position Purpose:

This role is 50% administrative, and 50% clinical care. The administrative responsibilities include oversight of integration and coordination of all pharmaceutical care services provided by Affinia Health Network. Develops and implements standards, programs, policies and procedures that guide and support the provision of pharmaceutical care services throughout the Regional Affinia Network. Serve as a liaison and representative with payers and governmental programs related to pharmacy coverage and services. Support the Triple Aim of Better care for the patient, better health for the community, and lower cost. Establishes approaches and methodologies which enable identification of strategies for improvements in pharmacy services. Identifies emerging trends and patterns that drive clinical pharmacy opportunities, and develop programing to address those needs.

Clinical responsibilities include providing pharmaceutical care to patients in assigned areas of practice, focusing on patients in value based programs. Supporting care teams with participation in care conferences and systematic case reviews. Collaborate with clinical programs including care management, palliative care, transition of care teams and others and needed. Implements and maintains medication management programs as defined under medical staff approved protocols. Provides drug information, education and clinical services to enhance the effectiveness of drug therapy and minimize patient risk. Implement population health initiatives related to pharmaceutical safety, cost and effectiveness.

Job Description Details:

Education

Minimum: Doctor of Pharmacy degree.

Credentials/Licensure

Minimum: Current State Pharmacy License. Current State of Michigan driver's license and proof of insurance. BLS required within 60 day of hire.

Related Experience

Minimum: Two years of ambulatory pharmacy experience, or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency.

Preferred: PGY-2 Ambulatory care pharmacy practice residency.
