About this job

The Client Care Coordinator is responsible for the coordination of services between clients and caregivers for the implementation of schedules, adequate staffing and continuity or care to best meet the needs of the clients.  The Client Care Coordinator reports directly to the Office Manager.

Principal Duties Include:

  • Answer the telephone and greet the public
  • Demonstrate dependability
  • Assign caregivers to all open visitis at a minimum of two weeks in advance
  • Communicate schedule changes to clients and caregivers on a timely basis
  • Schedule caregiver staff for hours of service for new clients
  • Schedule caregiver staff for hours of service for open hours/shifts on existing cases
  • Provide consistency in caregiver-client scheduling
  • Provide caregiver back up for open cases
  • Maintain documentation on all current and past clients
  • Monitor telephony logs on a daily basis.
  • Timely copying and filing of appropriate caregiver and client documentation
  • Maintain professional relationships with clients, employees, case manager and referral sources; assuring clients and employees' confidentiality
  • Respond to and resolve client complaints
  • Maintain the strictest confidentiality of client, caregiver and agency information

Requirements Include:

  • Two years experience in an office setting, preferably in health or home care
  • Demonstrate proficiency with Microsoft Office (Word, Excel and PowerPoint), Windows, internet, scheduling systems, social media and other health care industry related software
  • Excellent written and verbal communicatin skills
  • Ability to listen and communicate clearly, fluently and diplomatically orally and in writing
  • Ability to remain flexible, resilient and calm
  • Present a well-groomed professional image
  • Ability to plan, organize, prioritize and accurately follow through in work activities with time constraints and interrruptions to meet deadlines, as well as, work independently with a mimimum amount of direction and/or supervision
  • Ability to generate goodwill for the organization and its management among staff, clients and referral sources
  • Demonstrate a strong commitment to client service excellence
  • Possess and maintain good physical and mental health, including current TB testing
  • U.S. citizen or evidence of valid Alien Work Permit
