Schedule: Part Time Evenings, 32 hours per week

Shift: 2p-10:30pm

Position Summary

Provides supervision and assistance to the residents in the activities of daily living, responds to emergency calls and notifies the staff nurse of changes in resident's condition.

Minimum Education Requirements

High School Diploma or GED

Minimum Experience Requirements

Previous experience working in a senior community preferred.

License/Certifications

* Colorado QMAP Certification (Qualified Medication Administration Person) required within 90 days of hire.

* Cardiopulmonary Resuscitation (CPR) and First Aide certification must be obtained within 90 days of hire.|

Certifications must remain current following the initial 90 days of employment.

Position Dutites (Essential Functions)

Direct Care - 60%

* Provides resident care and services as assigned and, as provided for in the care plan, to assure the safety and comfort of residents, including but not limited to, assistance with bathing, shaving, dressing, laundry, escorting residents to meals and social activities, tidying their apartments, assisting them with their activities of daily living, taking vital signs, doing peri-care, skin care, oral care, reminders of or assistance with toileting, appointments, and help with using their oxygen.

* Assists residents with observance of physical and medical precautions following a hospital stay, therapy treatment or a life-threatening event. Must be able to assist residents physically throughout the shift.

* Maintains a professional manner at all times when interacting with residents or their families or visitors.

* Follows all safety guidelines, including OSHA health and safety rules and regulations, and other safety procedures.

* Attends and participates in training and education sessions as scheduled, including staff meetings, in- services, and any other staff development opportunities.

* Always uses the safety equipment and procedures provided when performing hazardous tasks.

* Promotes resident and staff safety through the use of proper body mechanics, and through frequent rounds on assigned areas.

* Reports resident needs, status changes and safety issues promptly.

* Follows policies and procedures without prompting.

* Is qualified in accordance with regulatory guidelines and individual facility policy to assist with the safe administration of medication, proper documentation and monitoring of supplies.

Collaboration - 20%

* Works well as a team member, displaying flexibility with assignments and a willingness to assist co-workers.

* Communicates in an effective and timely manner to appropriate people regarding resident needs and services, departmental issues and family concerns, through consistent verbal or written follow-up.

* Reports to and assists the LPN or charge person on the shift, as needed.

Informatics/Technology - 20%

* Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the

patient/resident care setting.

* Thorough and confidential maintenance of patient/resident charts and records.

* Document ongoing status, interventions, patient/resident response and outcomes in an accurate and timely manner.

* Use information technology to communicate, manage knowledge, mitigate error, and support decision-making.

Physical Requirements

Medium Work - exert/lift up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly.
