Position Summary

Provides direct and indirect nursing care under the direction of a RN and in accordance with policies and procedures. Continually monitors patients on telemetry by interpreting and recording ECG rhythms and reporting pertinent changes to appropriate personnel. Responsible for accuracy of reports and error reconciliation. Troubleshoots equipment problems and communicates needs to leader. Assists the healthcare team in coordinating, prioritizing and communicating patient, physician and staff needs. Assumes personal accountability for organizational mission and core values.

Minimum Education Requirements

* High School Diploma or GED

* Graduate of a Certified Nursing Assistant Program

Minimum Experience Requirements

* Six months previous CNA experience

* Previous Telemetry experience preferred

* Ability to read, write, speak, and understand English

* Basic computer skills Previous telemetry experience, preferred

* Previous nursing unit secretary or medical office experience, preferred

License/Certifications

* Current state CNA license for applicable state of employment

* Current Basic Life Support (BLS) certification from American Heart Association

* Certified Rhythm Analysis Technician (CRAT) from Cardiovascular Credentialing International (CCI), preferred

* Successful completion of ECG exam annually

Position Duties

* Assists with safe and appropriate delegated patient care tasks which are directly supervised by a RN. *

* Provides person-centered care with sensitivity and respect for the human experience in a collaborative fashion with other team members. Assists care team in achieving daily patient and family goals. *

* Provides quality patient care, including comfort and hygiene measures, preventative skin care, safety measures, transport, and nutritional and elimination needs. *

* Reports patient status changes and needs to RN accurately and in a timely manner. *

* Demonstrates effective use of clinical strategies to insure patient and associate safety. *

* Communicates concerns regarding hazards or errors related to patients to the health care team. *

* Uses Situation, Background, Assessment, and Recommendation (SBAR) communication techniques. *

* Accurately documents vital signs, input/output, nutritional intake, personal hygiene, weights and other patient care performed and reports findings promptly to RN. *

* Demonstrate working knowledge of telemetry monitoring system. *

* Responsible for observation and documentation of patient's cardiac rhythm: every shift, when an arrhythmia occurs, and on each new patient as they are placed on telemetry. RN to co-sign patient's documented rhythm. *

* Recognizes and notifies RN immediately when life threatening arrhythmia occurs. *

* Promptly responds to all monitor alarms according to department/hospital policy. *

* Documents all communication to care givers. *

* Documents rhythms strips according to department/hospital policy. *

* Maintains patient EKG records in systematic organized fashion. Files EKG reports onto charts as needed. *

* Documents reason when patient is temporarily or permanently removed from telemetry and if/when monitoring resumes. *

* Ensures central monitor is never left unattended. *

* Ensures all patients on telemetry are connected to central monitor and alarms are appropriately set. *

* Maintains inventory and replacement of telemetry supplies. Assures that all transmitter inventory is accounted for each shift. *

* Cleans transmitter after each patient use. *

* Helps to maintain equipment in good working order; notifies biomed of malfunctioning equipment. *

* Effectively uses time, personnel, equipment and supplies to provide high quality, cost effective patient care and monitoring. Organizes and prioritizes work to be completed within assigned timeframe to promote consistent patient comfort. *

* Maintains thorough and confidential maintenance of patient charts and records.

* Promptly triages and responds to patient call lights. *

* Utilizes computer technology to access and exchange data, i.e., lab results, interdepartmental communications, etc. *

* Maintains non-EMR patient records as needed to ensure availability of accurate patient information. *

* Responsible for inputting daily and special charges as requested. *

* Performs clerical duties, maintains department/unit logs and coordinates services as needed. *

* Greets patients/visitors and directs/escorts to the desired location. *

* May be tasked with rounding on patients throughout the shift asking if patients or visitors have any needs. *

* Ensures that work environment and patient rooms are neat and orderly to enhance unit and hospital image. Maintains and stocks nursing supplies. *

* Serves as a patient advocate. *

* Uses standards of care, hospital policies/procedures and regulatory guidelines to guide work. *

* Demonstrates ethical, legal and professional behavior. *

* Communicates in a clear, courteous manner with patients, families, visitors, coworkers, physicians and other departments. *

* Consistently demonstrates flexibility as evidenced by floating to other areas and is agreeable to work flexible schedules including weekends, holidays, evening and night hours as necessary to meet the needs of the patients and department. *

* Assists with quality improvement, safety and regulatory functions as directed. Participates in process change to improve practice. *

* Assists in training other nursing support staff, volunteers, students and other hospital personnel. *

* Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care setting. *

* Demonstrates effective time management and is proactive in prioritizing workload and meeting productivity requirements. *

Physical Requirements

* Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
