Creative Foundations is seeking Team Members to provide direct care support to individuals with developmental disabilities. Our mission is to provide individuals with Independence, Choice, Opportunity, and Security.
The Creative Foundations Supportive Living Program allows individuals with disabilities to live as independent as possible in the community. The level of services an individual requires is based on individual abilities and working through a team process. Services under Supportive Living include, but not limited to:
- House Cleaning and Maintenance
- Grocery Shopping
- Medical Appointments
- Socialization - Living Life!
- Personal Hygiene
- Behavior Supports
- Budgeting
The shifts we are primarily looking for are evenings and weekends. We also have overnight shifts available called On/site-On/call where you have the ability to sleep at the site and get paid to be there for health and safety.
We can train, so experience is not required. If you are a caring, dedicated, and all around nice person - we would love the opportunity to provide you with a lasting career! Great pay, benefits including paid time off and medical insurance.
Some Minimum Qualifications include:
- Current driver's license
- Proof of Minimum Age of 18 years old
- Proof of current automobile insurance listing applicant as a driver on the policy with state minimum liability amounts.
- Proof of high school diploma or G.E.D.
- Reliable automobile to transport individuals and able to pass inspection by Creative Foundations and the Ohio Highway Patrol.
- Satisfactory driving record from the Ohio BMV with no more than 5 points or citations over 9 miles an hour
- Satisfactory criminal background check.
