JOB DESCRIPTION

This is a professional nursing position responsible for Care Coordination to children with chronic illnesses. The professional occupying this position shall be assigned clients who receive services of specialty care and require contact to ensure the child receives optimal care. The professional may also be assigned to assist clients with psychosocial needs. Professionals occupying this position shall carry caseloads between 400-500.

This position reports to the Senior Registered Nurse Supervisor. Care Coordination activities are targeted to champion excellence in the delivery of health care for children with special health care needs. The professional exhibits effective communication, courtesy, respect and understanding when dealing with clients, the public, coworkers and staff of other community agencies. Responsibilities include but not limited to:

Conduct care coordination assessments of children with special health care needs. Develop comprehensive assessments and care plans in conjunction with the child, family or other disciplines and providers. Coordinate and document in PM/CC services for the children/families, including multiprogram staffings as indicated and follow-up on service provision.

Follow up on service provisions and monitor the child's/family's progress toward accomplishing treatment goals. Document updates in Electronic Health Record.

Provide supportive counseling and guidance to patient and family. Assess community resources and identify gaps in services, in order to coordinate patient/family needs.

Provide education for child's care, procedures, and child development to patient/parents in clinic and out of office setting, if applicable.

Works in conjunction with Ped-I-Care and Med 3000 to coordinate prescribed patient services after review of physician's assessment and treatment plan.

Employee will be in compliance with practice guidelines including preventative health care, continuity and coordination of care and Hedis performance measures.

In conjunction with physician and social worker evaluate need for protective action for abused/neglected children. Referral is made to appropriate agency for follow up.

Performs other related duties as required, so that the total needs of the child and family are met.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIREMENTS

Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 and three years of professional nursing, psychiatric nursing or nursing instruction experience. A bachelor's degree from an accredited college or university in nursing or nursing education can substitute for one year of the required experience.
