Responsibilities/Qualifications (Career Site Posting)

Has knowledge and understanding of daily assignments and unit activities.1.1 Takes assignment from charge nurse.1.2 Answers patients' call lights in a timely manner to determine needs and responds appropriately by fulfilling needs within the scope of job description or reports to the nurse.1.3 Communicates with CNA's and NA's from preceding shift concerning patient needs.1.4 Organizes and prioritizes patient care.1.5 Performs designated tasks and completes delegated assignments.1.7 Assists in admission transfer and discharge procedures.1.8 Performs and documents Hourly Rounding.Performs routine tasks and simple treatments under the direction of a registered nurse.2.1 Takes and records temperature, pulse, respirations, blood pressure, and patients stated level of pain on all assigned patients at the beginning of each shift. Takes and records vital signs more frequently as assigned. Reports any significant findings to the nurse2.2 Obtains weights on patients as ordered.2.3 Performs fingerstick blood sugars, records results in medical record, and reports abnormal results (less than 80 or greater than 120) to the nurse.2.4 Empties wound suction/drainage devices at 0600, 1400, 2200 and prn; records amount on graphic record.2.5 Applies and refills ice packs, refills K-pad units and humidifiers, and place patients in the CPM.2.6 Prepares and assists patients with Sitz baths as ordered.2.7 Retrieve specimens, label them correctly and delivers them to the Unit Secretary.2.8 Apply and removes restraints according to the current Mercy Health Center Policy. Record on the restraint flowsheet all activities and observations according to the Restraint Policy.2.9 Performs bladderscans and record finding according to policy.2.10 Changes telemetry leads daily.2.11 Turns and encourages deep breathing and coughing by post-operative patients every two hours. Provides personal hygiene and comfort measures to patients as directed.3.1 Provides complete/partial baths or showers, including oral hygiene.3.2 Provides fresh ice water at least once per shift.3.4 Records hygiene and comfort measures on graphic record.3.5 Assists with bedpan/urinal, bedside commode, or to the bathroom as needed. Documents output in medical record.3.6 Performs catheter/perineal care every shift and documents this activity.3.7 3.8 Changes and/or cleans ostomy bags as needed records any output on the graphic record per policy.Assists in activity.4.1 Maintains proper body alignment and activity by turning non-ambulatory patients every two hours and documenting position in medical record.4.2 Assists patient to chair or ambulates as ordered.4.3 Encourages patients to use the Bedside commode or assists transferring the patients so they can use the toilet in their rest room.Assists in the maintenance of adequate nutrition/hydration.5.1 Serves between meals supplements/snacks as ordered.5.2 Assists patients with meals, feeding if necessary.5.3 Records type of diet and percentage of food eaten on the graphic record.5.4 Correctly follows patients' diets as ordered.Demonstrates effective and accurate verbal, and written communication.6.1 Promptly reports all signs and symptoms indicating any changes in the patient's condition to appropriate nurse immediately(complaints of pain, response to surroundings, changes in alertness, unusual skin redness or tears, etc.)6.2 6.3 Reports off to charge nurse before leaving assigned area.6.4 Records care and other pertinent data according to organizational policies and procedures.6.5 Communication is timely, accurate, concise, complete, and appropriate.6.6 Creates a positive climate for communications.Maintains a clean and safe environment for patient and self by:7.1 Emptying and properly bagging linen and trash and storing it according to hospital policy.7.2 Cleaning bedside and overbed tables and straightening/changing bed linen prn.7.3 Removing unnecessary equipment from room.7.4 Cleaning and properly storing bedpans, urinals and bedside commodes.7.5 Uses proper hand washing techniques and uses standard precautions at all times.7.6 Complies with isolation procedures.7.7 Keeps bed at low position, call light within patient's reach, and alerts patient and staff to fall risk.Demonstrates knowledge of special needs and behaviors of the type of patients that are typically assigned to the unit in which they work8.2 Demonstrates ability to provide care based upon physical, psycho/social, cultural, safety, and other age related factors as appropriate for their patients.8.3 Demonstrates the ability to obtain and interpret information as appropriate for the patient.Promotes professional and organizational development by completing the following:9.1 Attends mandatory in-services and/or completes mandatory lap packets in a timely manner.9.2 Maintains current CPR certification.9.3 Assists with orientation of newly employed personnel and CNA students.9.4 Attends 75% of the unit's staff meetings.9.5 Performs high/low glucose test to maintain active scanning capability for fingerstick blood sugars.Provides unit support activities as necessary.10.1 Maintain a clean, neat and safe environment in the Nurse's Station and the Galley.10.2 Performs other clerical duties as assigned by nurse.10.3 10.4 Transports supplies from clean holding, unit secretary desk, and/or supply rooms as needed.10.5 Transports dirty equipment and meal trays that miss the return cart to soiled holding.

Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications:

Education: Must have High School diploma or GED. Certifications: Current BLS certification required. Required certification must be current before assigned to care for patients independently.Other: Must be familiar with procedures involved in patient tasks. Completes orientation through Educational Resources before beginning clinical orientation on the nursing unit.Preferred Education: A nursing student that has obtained certification letter form a faculty member that verifies completion of at least the first two semesters of an approved nursing school; or Graduate of a nursing program who is waiting to take or retake state boards.Preferred Certifications: Currently a Certified Nurse Assistant or an EMT in the state of Oklahoma.
