A competent clinical dietitian who coordinates and provides full range of nutritional care services integrated with other facets of health care to all patients that promotes and aids in the restoration of health. Completes accurate and appropriate clinical nutrition assessments, reassessment and nutrition counseling to patients/families according to established standards of care, individualized for each patient and executes nutrition care plans and interventions, while working with other care teams to achieve goals, positive outcomes and patient/family's values and goals. Provides effective patient teaching. Is a participative member of the clinical nutrition and interdisciplinary care teams.

Nutrition Screening

Accurately completes routine nutrition screening to identify patient at risk for malnutrition.

Nutrition AssessmentAppropriately assesses diagnostic data (diagnoses, anthropometrics, biochemistry, pharmacology, immunology) as well as patient's history, (medical, surgical, diet) to formulate and document an individualized Medical Nutrition Therapy care plan, including but not limited to oral supplementation, enteral nutrition and/or parenteral nutrition as needed using the Nutrition Care

Nutrition Follow-upAppropriately follows up to modify, update and document progression of Medical Nutrition Therapy plan based on changes in the patient's medical condition, desired outcomes, patient comprehension and anticipated compliance using the Nutrition Care Process.

Nutrition Education, Patient, Co-worker and Community: Provides appropriate nutrition education on a variety of disease processes (in an inpatient/outpatient setting) by evaluating the patient/family's knowledge to identify educational needs and communicate an individualized diet prescription and rationale. - May conduct monthly Cardiac Rehab classes. - May provide education for monthly Diabetes Support group(s). - May provide education for prenatal classes. - Maybe required to give in-services to other hospital departments. - Maybe required to participates in health fairs, T.V. presentations, newspaper interviews and other community relation events as assigned by the Director of Nutrition/Clinical Nutrition Services.

Enteral Nutrition ExpertiseDemonstrates knowledge of enteral nutrition (EN) by recommending formulas according to rationale for use and patient tolerance, verifying access/formula/fate, entering accurate orders when consulted and communication of EN plan to nursing.Total Parenteral Nutrition (TPN) Expertise: Demonstrates knowledge of TPN by recommending individualized and accurately calculated TPN formulas (fluid, content, base nutrients, electrolytes, vitamins, trace minerals and insulin), verifying access/formula/rate, entering accurate orders when consulted, and communication to TPN plan to nursing.Communications:

* Interacts proactively and collaboratively with co-workers, physicians, nursing, care team and other heath professionals to provide a multidisciplinary approach for patient care.

* Refers patients to other resources/healthcare disciplines such as, but not limited to, outpatient diabetes programs, psychologist, social worker, WIC, Meals on Wheels when needed.

* Acts as nutrition resource for the community. Participates in annual health fair and other community projects through Community Relations.Work Organization and Productivity: - Participates in the development of an effective monthly schedule, preferably one (1) month in advance, for daily, weekend and vacation coverage.

* Manages daily work routine by prioritizing and dividing patient care activities in order to use time and resources efficiently, taking into account skill mix, patient load, acuity and department guidelines.

* Remains flexible regarding schedules, job duties and the needs of others.

* Communicate with other co-workers to assist with the completion of tasks as needed.

* Documents daily productivity, in-services, classes and speaking seminars.

Teamwork: - Builds and maintains positive work relationships with co-workers, physicians, nursing, clinical nutrition team, other hospital departments. - Participates in multidisciplinary team for patient care and participates in wellness, ethics, discharge planning, and wound care committees. - Communicates and listens well Compliance: - Ensures compliance by complying with Hospital policy and procedures, Medicare requirements for nutritional assessment and therapy of renal patients and provides education to renal nursing staff. - Maintain a high professional profile. - Ensures compliance with HIPAA, all Federal, state and local agencies as well as third party surveyors.

Qualifications:

Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications:

Education: Bachelors degree in Nutrition meeting requirement for registration by the, The Commission on Dietetics Registration/recognized by The Academy of Nutrition and Dietetics. (required) Must obtain 15 CPEUs annually to maintain registered dietitian credential or cumulatively 75 hours within a 5-year period. (required)

Licensure: Licensed and Registered Dietitian. Credential recognized by The Commission on Dietitics Registration. (required) Must be registered with the State of Missouri or eligible to receive Missouri license. RD/LD Registered Dietitian, Or Registry Eligible - RD Within 6 Mo/LD Within 3 Months Of RD MO and LD Licensed Dietitian Within 3 Months Of Registry

Experience: 1. One year minimum as a clinical dietitian working in a clinical nutrition practice area. (required) 2. Be able to work independently with little supervision.

Other: Must be willing to learn EN/TPN
