Position Summary
Assesses, plans, implements and evaluates strategies for the management of the assigned population around a
continuum of care. Establishes monitors and analyzes the effectiveness and appropriateness of tools and processes
necessary for case management. Tracks quality, satisfaction and cost outcomes and then responds to those outcomes
appropriately.
Minimum Education Requirements
Associates of Nursing required
Bachelors of Nursing (BSN) or Masters of Nursing (MSN) preferred
Minimum Experience Requirements
2 years experience with Long Term Care ( LTC)
2 years experience in a staff development role.
Leadership/supervisory experience preferred
License/Certifications
Current CO Registered Nurse (RN) License
Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for
facilities with an Automated External Defibrillator (AED) and preferred for facilities without an AED.
CPR Instructor preferred
Position Duties (Essential Functions)
Staff Development- 50%
Research, plan, organize, and/or conduct all training programs,
seminars, and in-services for all facility personnel, and determine
the effectiveness of each. Track and maintain attendance log of
all associate training. Maintain library of videos and other training
aids. Organize and facilitate new associate orientation. Plan,
organize, and conduct any specialized training, e.g. dementia,
volunteers, safety, OSHA, required by facility as well as maintain
any related documentation. Plans for the provision of care in the
areas of responsibility, including, strategic and program planning,
and policy and procedure development while adhering to the
mission of the facility. Actively participates in strategic planning
and program development. Participates and provides leadership
for regulatory requirements. Will serve as clinical educator and
ensures that competencies are current.
Quality- 20%
Responsible to set-up, monitor and report on Nursing Indicators.
Follow-up on the monthly pharmacy report making corrections as
needed. Implements and participates in effective, ongoing
programs to measure, assess, and improve the quality of care
and customer satisfaction including but not limited to restraint
(including fall occurrence reports) and pain management
programs. Promotes an environment conducive to safe and
quality care. Identifies and participates in sentinel event reviews,
and participates in necessary revisions to policy and practice to
reduce or alleviate the chance for error in the future.
Direct Care- 10%
Assesses the clinical and psychosocial needs of the resident
along the continuum of care. Responsible for admission and
quarterly assessments to include, but is not limited to, use of
restraints, use of siderails, fall risk and skin assessment. Prepare
home medication information sheets for discharge. Provide direct
care as appropriate.
Coordination & Direction- 10%
Will work in conjunction with the Director of Nursing and MDS
Coordinator. Review MAR's and TAR's on a quarterly basis.
Collaborates with the nursing and multidisciplinary health care
team to ensure that residents/patient needs are being met.
Coordinates and integrates human and material resources to
support and meet the care needs in the areas of responsibility,
while supporting the overall organizational goals and mission. Will
assist Director of Nursing in writing, reviewing, and revising
policies and procedures. Will share charge nurse responsibilities
when needed. Will rotate weekly and weekend call. Provides
direction of services through organization, coordination, support of
staffing programs, and competent staff development through
education/orientation, communication and support. Participates
and provides leadership in education / orientation programs with
the nursing care group.
Finance & Stewardship- 10%
Review all pharmacy bills and report to designated leadership.
Follows OSHA guidelines and facility safety programs to ensure a
safe environment. Responsible for the proper care of all property,
equipment, supplies, and financial assets involved in the
performance of the job. Assures effective and efficient financial
utilization of human and capital resources, while supporting
effective stewardship, and contributing to the financial health of
the entity. Supports improvements to support associate
