This is a weekend position and the required shifts are Friday, Saturday, and Sunday 8 am - 4:30 pm. Please be sure you can commit to this schedule each week.
Responsibilities Provides professional skilled nursing care to patients which includes initial nursing assessment and assessing patient mentally and physically on each visit and reporting to physician when appropriate. Develops plan of care in collaboration with physician's orders and establishes realistic and measurable goals, updating goals when necessary. Evaluates effectiveness of the care plan frequently. Performs procedures and treatments with proper techniques and has adequate knowledge of medications' actions and effects. Complies with standard nursing policies and procedures. Uses equipment properly to give treatments. Practices cost containment with time and supplies. Supervises LPN and aide as applicable. Instructs and explains plan of care to patient and family, teaching as needed to encourage self-care. Uses additional services and community resources as appropriate. Plans and prepares patient and family for discharge. Consults with Physician, patient, caregiver, and other health care professionals regarding coordination of treatment, goal development and response to treatment. Reports on patient at patient care conferences. Identifies need for other disciplines. Documents accurately and timely patient treatments, care, evaluation, and progress, and communicates with other staff members. Participates in Quality Improvement activities, inservices, patient care conferences, workshops and seminars. Assumes responsibility for continuing education and maintaining current knowledge. Assess home situation for possible patient abuse. Assists with orientation and training of other staff members.
Position Qualifications:
* We require a nursing degree from an accredited school, BSN preferred.
* Licensure: Current state licensure. Valid drivers license.
* Basic life support training through American Heart Association
* Must have at least 1 year of nursing experience; strongly prefer experience with homecare or acute care.
* Must be confident in your IV skills.
* EMR experience helpful but not required.
* Basic computer skills and a working knowledge of Windows, Word and Outlook.
* Other: A broad range of assessment skills for a multiple range of medical diagnoses and chronic health issues is needed. Critical thinking skills are especially important for nurses working in the patients homes away from the resources of a hospital setting. Being a good listener and effective teacher are also important to help plan needed care and education. Written and verbal communication skills are helpful in the coordination of care among services including physicians, physical therapy, occupational therapy, speech therapy, social worker, and home health aides. Employee has regular contact with agency staff members, physicians, hospital staff and general public. Employee regularly handles confidential information.
