Job Summary:

Provides primary and/or specialty nursing and/or medical care in an express clinic as a nationally certified licensed healthcare provider. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Certifications may include: acute care, adult, family, pediatrics and women's health. Provides low acuity patient care with autonomy, after hours in an express clinic

Responsibilities:

* Assess health status by obtaining relevant health and medical history.

* Performs a physical exam based upon the age and patient history Identifies significant health and medical risk factors. Documents findings.

* Orders, interprets and evaluates laboratory test and other diagnostic studies to assess patient's clinical problems and health care needs.

* Develops a diagnosis by analyzing and synthesizing collected data.

* Develops and implements plan of care in collaboration with the physician, patient and significant others.

* May perform diagnostic or therapeutic interventions consistent with certification, law and Cleveland Clinic privileges.

* With certificate to prescribe, may prescribe and monitor medications according to applicable state APN formulary and CCF policy.

* Develops and implements patient education plan. Educates members of the healthcare team. Provides health education and utilizes community resources as appropriate to promote optimal health.

* Other duties as assigned.

Education:

* PA: Graduate of an A.R.C.-approved Physician Assistant Program. Master's degree in Physician Assistant Studies and/or applicable state licensure through the State Medical Board for certificate to practice.

* CNP: Effective 05/01/2014: MSN required upon hire or within 5 years of hire for external hires and internal hires moving from a non-APN licensed role to an APN licensed role.

Certifications:

* PA: Must have Prescriptive authority or eligible to apply for licensure to prescribe from the State Medical Board within the first 6 months of hiring. If candidate has held a license to prescribe within another jurisdiction and needs to obtain a Master's degree in order to apply for CT prescribe from applicable state, the candidate will be subject for review for eligibility for hiring within our system.

* CNP: Licensed as RN in applicable state , national certification as nurse practitioner, current certificate of authority by applicable state Board of Nursing, certificate to prescribe preferred.

* Basic Life Support (BLS) through American Heart Association (AHA) Required.

Complexity of Work:

* Demonstrated competency and the ability to perform advanced assessment and diagnosis.

* Must be able to critically think, assessments of low acuity patients with autonomy.

Work Experience:

* Minimum one year experience.

* Good verbal, written and interpersonal communication skills.

* Demonstrated competency and the ability to perform advanced assessment, diagnosis, treatment plan development and invasive procedures as per clinical privileges.

Physical Requirements:

* Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

* Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

* Follows standard precautions using personal protective equipment as required.
